Carotid Artery Disease

Cerebrovascular Disease Experts at CIS treat patients with cerebrovascular disease which effects blood vessels in the brain. CIS physicians assess patients to determine the cause of the stroke, and if an artery is blocked, all treatment options are considered.

CIS physicians have pioneered the use of carotid angioplasty and stenting to treat carotid artery disease. This minimally-invasive treatment method is a groundbraking alternative to surgical options that previously prevailed as the only source of treatment. Our physicians evaluate each patient on a case-bycase basis to determine the best course of treatment.

Peripheral Artery Disease Venous Disease Our expert physicians also diagnose and treat venous reflux disease with minimallyinvasive procedures, resulting in less pain and quicker recoveries for patients. The CIS vein centers offer services such as duplex ultrasound, radiofrequency (RF) ablation, endovenous laser therapy, sclerotherapy, and phlebectomy, all of which are performed with a clinic setting.

CIS is globally recognized for high-profile advancements in the development of non-surgical treatments of peripheral artery disease (PAD). Through the use of interventional procedures to eliminate the need for amputations, we are able to save limbs and lives. With the institute positioned on the leading edge of today’s cardiovascular technology, our patients can rest assured that their individual cases will undergo thorough review, and all available treatment options will be considered.

CIS Virtual Care Center Programs CIS Telecardiology CIS offers telecardiology services to remotely-located hospitals, allowing patients to receive world class cardiology care without leaving their community. CIS telecardiologists provide consultations in the emergengy room and on the department floor, improving access to care and reducing hospital costs. By partnering with CIS, hospitals can enhance patient and physician satisfaction and improve continuity of care, subsequently leading to better patient outcomes. Success is measured by response time in the ER, patient satisfaction and the positive financial impact on the hospital.

CIS Cardio@Home Cardio@Home utilizes state-of-the-art technology to remotely monitor patients at home, providing consistent, continual care when needed. The Cardio@Home care team features physicians, nurses and hospital case management employees that all work together toward the common goal of improving patients’ health and well-being. Family members are also integrated into the care plan to provide extra support. This remote patient monitoring program reduces risk of readmissions and improves quality of life for patients, who can have peace of mind knowing that their care is consistently being monitored when leaving a hospital.

12

CARDIOVASCULAR INSTITUTE OF THE SOUTH

Rhythm Management Remote monitoring has become the national standard of care for patients receiving cardiac rhythm management devices. It allows us to constantly monitor patients and ensure that they are receiving the very best care. Studies show that remote monitoring increases survival rates and reduces ER visits, re-hospitalization, length of hospital stay and office visits.

Coumadin Remote Patient Monitoring Our Coumadin monitoring system offers patients the capability to have their Coumadin checked in the comfort of their own home. This saves time, provides consistency and frequency of reporting and offers easy integration with technology and electronic health records.

Chronic Care Management OMS C3 allows a CIS provider to connect with patients via non-face to face visits such as phone calls, emails, text messages and videos to help patients monitor their chronic conditions. OMS C3 is a way to increase interactions with our patients to improve their overall care. In addition, more frequent patient/provider interactions lower the cost of care, mortality rates and hospital readmissions, while increasing patient compliance, enhancing documentation and medication reconciliation, and improving patients’ quality of life overall.

The CIS Model: A Key Component of our Success The CIS model begins with an understanding of physician practice and hospital cultures. It is critical that both entities are respectful of each other’s cultures and that we never lose focus of our mission to offer the best care possible to our patients. An integral part of CIS’s success has been our ability to attract exceptional talent. Once we identify and employ team members, they are integrated into the CIS culture by spending a substantial amount of time with our corporate team and with our experienced hospital partners. We believe that hiring the best people, immersing them into a patient-centered culture and then focusing on the tasks at hand invariably leads to lasting success.

As the healthcare community begins to assimilate data and create long-term strategies for addressing population health management, a crucial nexus will come with the alignment of physicians and hospitals. The CIS co-management model has an added benefit of appealing to today’s new generation of physicians who are trained in, and also believe in, the hospitalphysician alignment model. By becoming part of this model, these physicians get to participate in hospital projects, committee meetings, strategic planning and other hospitalbased events with a feeling of inclusion and ownership. In the long term, these activities build lasting relationships that benefit the patients, the hospital and the community.

In creating a successful co-management model, all parties have to agree to devote their time and expertise in the development of common goals, objectives and metrics. The vision of all parties must be aligned in order to facilitate the development of meaningful metrics. CIS’s many agreements often have very similar metrics in place, as most hospitals currently have consistent goals to achieve. Quality is always paramount to our hospital partners, closely followed by supply cost control and efficiencies.

Cardiovascular Solutions Designed to Help Health Systems Thrive

CIS offers solutions in alignment with your health system’s strategic goals for cardiovascular services. For hospitals worldwide, cardiovascular programs represent a cornerstone of financial well-being. Cardiology services alone can be responsible for more than 40 percent of a facility’s contribution margin. However, it can be difficult for health systems to offer superior patient care and to recruit and retain physicians in competitive markets while operating under strict budget requirements. Adding to the problem is the fact that there is a growing shortage of cardiologists worldwide. How can hospitals continue to thrive while facing these challenges? CIS recognizes the traditional strengths of hospitals and the modern challenges they face, and we can help your organization launch and maintain a cardiology practice, develop a cardiovascular service line or invigorate its existing program. We offer customizable management solutions designed to meet the needs of your facility, addressing various aspects of your cardiovascular program, from managing costs and evaluating opportunities, program growth and development, to exceeding all cardiology core measures.
