Step 1 All employees start SIH with a reduced monthly insurance premium. In order to keep your reduced premium and avoid a $200 per month increase in 2020, both you and your covered spouse must participate in baseline wellness activities, plus choose from a variety of additional activities to earn a total of 1000 points by December 15, 2019.

Step 2

Once you have your results, call the wellness department at 618-457-5200 ext. 67828 to speak with a health advisor for a 5-10 minute call to learn what *wellness programs will have the greatest impact on shaping a healthier you.

Step 3

Upon completion of the advisement call receive 25 points.

Diabetes self-management workshop This . 6 week workshop is for anyone living with type 1 or

type 2 diabetes or anyone who is a caregiver for someone with diabetes. 250 points

This 4 week education series focuses on you how to manage your diabetes. 250 points

This 8 week group program is anyone with a 25 BMI or above. It’s designed to help participants build healthy habits and reach sustainable weight-loss goals. 250 points

Courage to QuitÂŽ tobacco cessation program

This 7 week education program gives you the support and tools to quit smoking in a group setting. 250 points

Financial coaching and assessment Complete a quick assessment, discuss your results with a financial coach, and move toward a more secure future (by telephone or in person start anytime before September to complete). 250 points, three-session minimum

Go to the wellness portal to connect your step tracker to the wellness portal. Once your tracker is connected start turning your steps into points.

Step 2 Earn 100 points for each million steps up to 4 million steps and 400 points.

Four new classes in 2019 on a range of topics that will help you make lasting changes to your 1-hour live wellness classes health and improve your quality of life. Setting up annual physicals and recommended screenings will not only help you catch early signs of disease it will also provide a baseline of your current health to help you track your progress towards a healthier you in 2019.

Annual physical / Biometric Screening

Complete one annual routine physical exam with a primary physician. You should try to schedule this as early in the year as possible so that if your physician identifies any risk factors, you can develop an action plan to improve your numbers.
