Ejemplo: número de palabras inicial: 437 ABSTRACT Tuberculosis (TB) is one of the most pervasive diseases nowadays, with over one-third of the world population infected with Mycobacterium tuberculosis (Mtb) and 2 million deaths every year. After primary infection, only 5–10% of the infected hosts develop active disease, whereas a high percentage of the remainder experience latent infection. Anti-mycobacterial therapy remains the cornerstone in controlling TB. Given the increased number of immunosuppressed individuals at risk for infection and the rising prevalence of multi-drug-resistant (MDR) isolates, other therapeutic options for TB, such as immunomodulators, are becoming attractive. Host control of mycobacterial infection, in both human and mouse models, has been associated with the production of interferon (IFN) by CD4+ T cells. Interleukin (IL)-12 is known to be a crucial cytokine in the differentiation of IFN producing T helper 1 (Th1) cells. Many studies have established an essential role of exogenous administration of IL-12 (rIL-12) in the induction of Th1 response. The intermittent administration of rIL-12 through parenteral injection, can control mycobacterial growth efficaciously. However, these studies did not address whether continuous administration of IL-12 through the oral route is also able to maintain the Th1 response during early TB or to control advanced disease once the Th1 protective immune response has declined. Although the currently used licensed protocols of rIL-12 administration have been designed for parenteral administration, the availability of a suitable vehicle for oral delivery of pharmaceuticals would be highly beneficial. In this regard, plants represent a very attractive alternative. In this study we report the use of an edible crop, the tomato, with a construct bearing a single chain murine IL-12 [transgenic tomato (TT)–IL-12] gene. We employed crude fruit extracts for oral delivery of IL-12 in a murine model of pulmonary TB, aimed at studying its therapeutic efficacy in animals infected with drug-sensitive (laboratory strain H37Rv) or drug resistant bacilli (clinical isolate code 900). BALB/c mice were infected with any of the strains and treated with a daily oral dose of TT-IL12 crude fruit extracts. For the early H37Rv infection, TT–IL12 administration was started 1 day before infection and continued for 60 days. In the H37Rv or MDR late infection, treatment was started 60 days after infection and continued for another 60 days. In both phases of infection, TT–IL12 administration resulted in a reduction of bacterial loads and tissue damage compared with wild-type tomato (non-TT). The Th1 response was increased and the Th2 response was reduced. In the late infection, a long-term treatment with TT–IL12 was necessary. We demonstrate that TT–IL12 increases resistance to infection and reduces lung tissue damage during early and late drug-sensitive and drug-resistant mycobacterial infection.
