A New Direction in Healthcare Excellence

The year 2012 was a special one for PBMC Health, full of challenge, growth and change. We accomplished a number of major milestones in our continuing mission to serve the people of our communities. In doing so, we accomplished something else that makes PBMC Health proud to be part of this growing community of caregivers. We have changed people’s perceptions about the quality of healthcare service available in eastern Suffolk County. While many people already preferred our hometown brand of compassionate care, many more are now realizing they no longer need to drive west to obtain the most advanced medical technology or the highest safety ratings.

A New Direction in Healthcare Excellence

What was once a small community hospital has grown to become a leading-edge resource—a destination for people seeking the highest quality medical care supported by a comprehensive continuum of health and wellness services. The story of what we have accomplished and our plans for the future are featured throughout this report. As leaders, contributors and supporters, we are proud to present you with our first formal Annual Report as PBMC Health.

Giving a new name to our enterprise is a public declaration of our commitment to our new direction as a forward-leaning, responsive and proactive community service resource. What began in 1951 as a small community hospital serving the residents of Riverhead has responded to the growing needs of the region by expanding beyond the walls of the hospital to encompass a comprehensive and continuously growing continuum of care under the name PBMC Health. Focused on addressing the needs of the people and families we serve through all the stages of life, the PBMC Health continuum of care now includes:

A New Direction in Healthcare Excellence

…in Community-Based Care

…in Creating a Community of Care

Truly Delivering Care…to where it’s needed most: In its mission to provide the best possible health care services for the community, PBMC Health has grown from a one-location community hospital to a wide-ranging network of communitybased services, offices and facilities.

Of the community, for the community: The caregivers of PBMC Health are an integral part of the community. Many of us grew up in Suffolk County. Our patients are not just faceless statistics, but our family members, neighbors and friends. Peconic Bay Medical Center is our hospital of choice, not just to work and provide care, but to receive the best possible care as well.

In addition to Suffolk County’s largest and most comprehensive provider of Home Care services, the PBMC Health continuum of care now includes a growing network of primary care family medicine practices, communitybased specialty physician practices, community-based rehabilitation and laboratory services, and The Gertrude & Louis Feil Campus for Ambulatory Care, which will offer a growing range of services in a convenient Manorville location.

There are more than 1,200 of us working together as a single team to enhance the quality of care for the community, supported by hundreds of dedicated volunteers, generous donors and local business sponsors. Together, we are also improving the community’s quality of life.

The Heart of PBMC Health: The Main Campus As the center and foundational resource for the PBMC Health continuum of care, the main campus offers central access to leading-edge facilities that support all the other services and service providers of the continuum. Continuous investment in the facilities, capabilities and people of the Main Campus are essential to the continued growth of PBMC Health and our ability to serve the growing needs of the community.

PBMC Health Emergency Center: Providing expert care for 35,000 emergency room visits annually, our emergency department is a NYSdesignated Stroke Center, staffed by faculty physicians from the Stony Brook University’s Department of Emergency Medicine.

The accomplishments shown on these pages would not have been possible without the generosity of our donors, volunteers, hard-working staff members, physicians and community supporters.

Peconic Bay School of Radiologic Technology: PBMC Health’s highly regarded 24-month training and certification program for radiology technologists has now become affiliated with SUNY Empire State College and is the only hospital-based program in Suffolk County.

Peconic Bay Medical Center The centerpiece of the Main Campus and the heartbeat of PBMC Health, Peconic Bay Medical Center is the largest hospital system in eastern Suffolk County.

Peconic Bay Graduate Medical Education, Family Medicine and General Surgery Residency Program: As a major clinical campus for the New York College of Osteopathic Medicine, PBMC Health has expanded its graduate medical education programs to include residency training programs in Family Medicine and General Surgery.

A New Direction in Healthcare Excellence

New Soarian™ Electronic Health Records System: The advanced new Siemens system supports faster, more accurate diagnoses, while improving patient service and patient safety.

Peconic Bay Skilled Nursing and Rehabilitation Center and Pegasus House Palliative Care: One of the most highly regarded facilities of its kind on Long Island, our nursing, rehabilitation and palliative care programs have earned a Five-Star Quality Rating from the Center for Medicare and Medicaid Services (CMS), which identifies these facilities as “much above average” and received a 5-Star rating from US News and World Report.

The Kanas Center for Advanced Surgery: Widely considered among the top surgical facilities in the Northeast, the Kanas Center is now the surgical venue for two designated Centers of Excellence: Total Joint Replacement for Hip and Knee, and Bariatric Surgery. daVinci Si Robotic Surgery System: The renowned, minimally invasive surgical technique that reduces pain, blood loss and recovery time.

Going above and beyond... PBMC Health Community-Based Facilities & Services:

The Community’s First Choice for Family Care One of the major changes in direction for PBMC Health—and for health care providers throughout the nation—has been reaching out into the community to provide services that prevent, replace or reduce the need for hospitalizations. This starts with assuring that every family has access to the highest quality in primary care: every family deserves a family doctor. PBMC Health’s growing network of multi-specialty, family practice and urgent care locations continues to provide services for families throughout our region, acting as resources to guide our fellow members of the community to healthier lifestyle choices as well as to the best possible medical services and specialists if they are needed. With the national mandate for health care reform and the implementing of the Affordable Care Act, our focus on community-based primary care and family medicine will continue to grow in the coming years. Already serving the widest geographic area of Suffolk County of any home care agency, our Peconic Bay Home Health Services team now offers specialized testing and monitoring services as well as wound and ostomy care, post-op rehabilitation, and support for long-term disabilities. Reaching Out with Community-Based Resources To serve the community best, we are reaching out beyond our Main Campus to provide services where they are most needed and most accessible. Our current communitybased facilities include: sPBMC Health Multi-Specialty Practices Conveniently located private office locations make it easier for patients to consult directly with leading specialists. sPeconic Bay Outpatient Physical Therapy & Rehabilitation PBMC Health is focused on expanding community-based access to advanced-technology rehabilitation and therapy services. sThe Gertrude & Louis Feil Campus for Ambulatory Care Opening in Manorville in 2013, this multi-building medical campus will initially provide primary care, urgent care, pediatric and orthopedic services to a growing community. Every Vision Needs a Strong Foundation. The PBMC Health Foundation plays a critical fundraising role, providing resources for high impact initiatives that contribute to healthcare excellence. Since its establishment in 2004, the Foundation has been dedicated to raising private support to improve patient care, creating a model for successful publicprivate partnerships. An independent non-profit 501(c)(3) organization, the Foundation depends on the generosity of individuals, corporations and foundations to ensure that Peconic Bay Medical Center and the PBMC Health continuum can pursue its mission to provide: s!CCESSTOHIGH QUALITYHEALTHCAREFOR the community s#OMPREHENSIVE COMPASSIONATECAREFOREVERYONE s3TATE OF THE ARTTECHNOLOGYTHATADVANCESTHECARE of patients s3UPERBEMERGENCYCARE s6IGILANCEINHEALTHEDUCATION

Emilie and Michael Corey – As major benefactors to PBMC Health, Emilie and Michael Corey brought the vision and provided the support to establish our highly acclaimed palliative care program and built what is now our Pegasus House Palliative Care Center. With Emilie’s leadership as Campaign Chair, in 2011 PBMC Health launched an ambitious $13,000,000 capital campaign, Building Upon the Promise of Excellence. “Dear friends, We asked and you responded. Individuals, corporations, organizations and foundations have given generously to PBMC Health’s Campaign, Building Upon the Promise of Excellence. While the original $13 million goal is in reach, the on-going need is far greater. Meeting this challenge requires a remarkable philanthropic commitment and comes from the shared belief that we can provide better access to healthcare through new facilities; enhance therapies and treatments with state-of-the-art technologies; and deliver comfort and compassionate care from the best physicians, nurses and support staff. I am touched by your belief in our mission. We can do more together!”

Irwin and Flora’s lifetime contributions to the Hospital exceed more than half a million dollars. “What the hospital does for the community is beyond measure. There is no comparison – the care provided by PBMC Health far a more important cause to support.” ~ Irwin Garsten

The Gertrude and Louis Feil Family Foundation: The long anticipated ambulatory care center in Manorville will open its doors in late spring, thanks in part to one of the largest individual philanthropic donations PBMC Health has received. The Louis Feil Charitable Lead Annuity Trust contributed $5 million toward the completion of the ambulatory campus, and in recognition of the Feil Family’s generous philanthropy, the campus will be named, The Gertrude & Louis Feil Campus for Ambulatory Care. “You are able to help so many people so quickly that didn’t have access to healthcare because the facilities were not available. You are really providing support to the people in the community right away.” ~ Jeffrey J. Feil, CEO, The Feil Organization (on behalf of the Louis Feil Charitable Trust and the entire Feil Family).

Making a Difference

During the last year, the PBMC Health continuum provided care for more than 7,500 inpatients; 35,000 emergency room patients and 70,000 outpatients. The PBMC Health Foundation supported the system’s efforts with $7,741,000 in gifts, thanks to thousands of people in our community who made donations and volunteered their time.

PBMC Health Employees: The PBMC Health employees have been instrumental in supporting the Capital Campaign and reached a historic milestone by raising more than a quarter-of-a-million dollars to purchase a new Telemetry System and an MRI Crash Cart. “As an employee, I feel privileged to have had the opportunity to participate in this unprecedented campaign. Together, we raised more than $250,000! This has never been done in our hospital’s history, and we know that we must invest in ourselves before others will invest in us.” ~ Betty Ruth, Plant Operations

Fun with Purpose: Events Deliver Significant Fundraising Results Foundation events are an important way for community members to connect with and support PBMC Health while having some fun. Our three signature annual events held in 2012 offered something for everyone while raising a significant amount for PBMC Health: The Candlelight Ball raised $303,000 for program and education initiatives The Garden Festival raised $107,000 for Pegasus House Palliative Care The Black & White Gala raised $308,000 for emergency care initiatives Sources of Revenue The PBMC Health Foundation is grateful for the generous support of our donors. A Community of Care. The Vital Roles and Contributions of PBMC Health Volunteers and the PBMC Health Auxiliary PBMC Health could not fulfill its mission to serve the community without the hands-on support of our many generous and dedicated volunteers and Auxiliary members. They are members of the community from all walks of life who have chosen to become contributing members of our community of caregivers, and they help raise the quality of care our patients and their families receive in countless ways.

The kind, caring, smiling faces of our front desk volunteers are often the first point of contact for patients and visitors. Our hospital gift shop is entirely staffed by volunteers. Other volunteers serve in our Emergency Department, Skilled Nursing & Rehabilitation Center, Outpatient Physical Therapy & Rehabilitation Center, and in diverse clerical positions throughout the medical center. They provide vital services supporting professional staff members as well as providing comfort, care and assistance to patients, patient families and visitors.

The proceeds are donated to the hospital and have helped fund many important needs, including vital medical supplies, beds, rehabilitation equipment and renovations for special care units. And the work goes on: the Auxiliary has pledged $1,000,000 through the year 2020 to support the PBMC Health Foundation’s Building Upon the Promise of Excellence Campaign.

A Foundation for the Future

In its growth from a small, limited-service community hospital to a comprehensive multi-dimensional continuum of care, PBMC Health has responded to the growing healthcare needs of the entire region. As the population of eastern Suffolk County continues to grow and age, those needs will continue to expand. Through the efforts and initiatives of the PBMC Health Foundation, with the continued support of a generous and forward-thinking community of donors and stakeholders, PBMC Health is acting to anticipate those needs.

Challenge: Living Up to our Commitment to Continuous Improvement in Patient Safety and Quality of Care Continuous advances in technology, treatment protocols and evidence-based safety practices have made it possible for regional providers like PBMC Health to offer levels of quality comparable to those found in major urban centers such as Boston and New York City. Achieving those levels, however, requires a serious commitment in terms of staff competence and dedication as well as financial investment. The PBMC Health Response:

These emerging challenges for the future converge into a few broad categories.

sRecognition as a Leader in Safety PBMC Health has been rated in the top 2 in the Tri-State Area and in the top 3 in all of NYS for Patient Safety by a leading national consumer rating agency. s5-Star Rating for our Skilled Nursing Long recognized as the leading determining factor in hospital quality, PBMC Health’s nursing has been recognized by both US News & World Report and the federal Centers for Medicare and Medicaid Services as among the best in the nation. sCenter of Excellence Designations for Bariatric Surgery and Joint Replacement Centered in our award-winning Kanas Center for Advanced Surgery, our Bariatric Weight-Loss Surgery Program has received Center of Excellence recognition from the American Society for Metabolic & Bariatric Surgery (ASMBS), and our orthopedic surgery program has received Gold Seal recognition for total hip and knee replacement from the Joint Commission for Accreditation of Health Care Organizations. sA New, State-of-the-Art Progressive Care Unit The new unit will provide individualized care for medical and surgical patients whose needs are not serious enough for the ICU but too complex for the regular in-patient floor, in a setting designed to maximize patient and family comfort and be more conducive to patient recovery.

A New Direction in Healthcare Excellence

Challenge: A Continued Shift Toward Community-Based and Home-Based Care New surgical and other treatment techniques are reducing the need for and the length of many hospital stays. Comparative costs of care are also contributing to the growing demand for greater capabilities for community-based care.

Challenge: A Nationwide Shortage of Skilled Medical Professionals With an estimated 30 million newly insured patients soon to be increasing the nationwide demand for healthcare services, the competition for qualified family doctors, nurses, physician assistants and medical technicians is already intensifying. Working to End Childhood Obesity: A First For NYS! PBMC Health helps bring Project Fit America to our schools It’s a nationwide epidemic that is costing lives and causing healthcare spending to spiral out of control. In fact, obesity is considered by many to be both the leading and yet the most reversible cause of health costs and problems. Experts and advocates, including First Lady Michelle Obama, agree that the best place to start in addressing this epidemic is with our children—in our homes and our schools. Obese children are more than twice as likely to become diabetic compared to children of normal weight. PBMC Health invites you to join in the effort to address this vital issue. With community support, PBMC Health is helping launch the first ever Project Fit America program in New York State.

The start-up program will serve all five elementary schools in the Riverhead Central School District: s0ULASKI3TREET%LEMENTARY3CHOOL s2OANOKE!VENUE%LEMENTARY3CHOOL s2ILEY!VENUE%LEMENTARY3CHOOL s0HILLIPS!VENUE%LEMENTARY3CHOOL s!QUEBOGUE%LEMENTARY3CHOOL Approximately 2,765 students and their families will be involved in the program the first year. After that, the program will be selfsustaining and hopefully expanded to every school district in the PBMC Health service area. 23

How you can help

Five Keys to a Healthier Community

Your words and deeds can make a real difference in people’s lives Community support plays a vital role in helping PBMC Health and our hospital programs provide the latest advances in patient care, promote healing environments and foster community health. The professional efforts of our 1,200+ staff members and physicians would not be possible without the supportive efforts of the thousands of community residents—your friends, neighbors and family members—who contribute their time, energy and financial support. We ask and urge you to get involved in helping us improve the quality of health and life in our communities. You can help by making a financial donation; or by volunteering your time, energy and expertise; or by contributing to and becoming a member of the PBMC Health Foundation.

1. Take Care of Yourself You are your own first line of defense against disease. Eat healthy, sensible meals; get plenty of exercise and rest; and follow your family doctor’s advice about getting regular check-ups and preventive care.

2. Volunteer a Little of Your Time Even two hours a week can make a real difference in people’s lives while immeasurably enriching your own.

We can promise you rewards that cannot be measured in any bank statement. 3. Learn About Your Health Care Options Studies show that well informed patients have better outcomes. Keeping up with today’s rapid advances in medicine and the health care system is vital.

4. Ask Questions and Speak Up About Your Needs When it comes to your health or the health of someone you love, the only foolish question is the one that is not asked. Talk to your doctor or call us at PBMC Health. We want to make sure you understand your options and that we understand your needs. 5. Tell Us How We’re Doing Your feedback helps. If we have areas in need of improvement, we want to hear about it. And if you want to tell us we’re doing a good job, it makes us want to do even better.
