Phelps Memorial Hospital Center’s Cancer Program earns three-year accreditation with commendation from Commission on Cancer PAGE 8

Phelps Memorial Hospital Center has again been granted a three-year accreditation with commendation as a Community Hospital Cancer Program by the American College of Surgeons’ Commission on Cancer (CoC).

Initial accreditation was earned in the 2013 cycle. To be accredited, a hospital must have an oversight body that meets quarterly, hold multidisciplinary tumor board cancer conferences that meet regularly to review existing cancer cases, have quality improvement measures in place, and participate in community outreach.

Avraham Merav, MD, FACS, Chief of Thoracic Surgery and Medical Director of Surgical Services at Phelps, is the Cancer Liaison Physician responsible for providing leadership for Phelps’ cancer program and spearheading cancer-related activities. Having a Cancer Liaison Physician is another of the Commission’s requirements for cancer programs to be accredited.

“Accreditation by the Commission on Cancer is much more than just a badge of honor,” says Dr. Merav. “Achieving and maintaining accreditation assures our patients that Phelps meets all up-to-date national standards of cancer care.”

The CoC accreditation program helps facilities improve quality of patient care through various cancer-related programs focusing on the full spectrum of cancer care. These include prevention, early diagnosis, cancer staging, optimal treatment, rehabilitation, life-long follow up for recurrent disease, and end-of-life care. Patients who receive care at a CoC facility have access to information on clinical trials and new treatments, genetic counseling and patient-centered services including psycho-social support, a patient navigation process, and

a survivorship care plan that documents each patient’s care and seeks to improve cancer survivors’ quality of life.

Like all CoC-accredited facilities, Phelps maintains a cancer registry and contributes data to the National Cancer Data Base, a joint program of the CoC and the American Cancer Society and the largest clinical disease registry in the world. Data on all types of cancer is tracked and analyzed through this dababase and then used to explore trends in cancer care and to create national, regional and state benchmark reports. CoC-accredited cancer programs have access to information derived

from these reports, which helps the facilities with their quality improvement efforts.

Leadership of Phelps’ cancer program is provided by the multidisciplinary Cancer Committee made up of physicians from diagnostic and treatment specialties and non-physicians from administrative and supportive services.

The 20 members of the committee, who represent the full scope of cancer care and services, meet quarterly to ensure that the hospital’s cancer program meets all of the standards set by the CoC. In remarks to staff at Phelps regarding the accreditation, Dr. Merav explained that although establishing an accredited cancer

program was a team effort, he specifically wanted to “recognize former Cancer Program Coordinator Cherian Mathew, CTR, who was instrumental in obtaining this accreditation.”

One of the main requirements for accreditation is the Tumor Board Conference, a multidisciplinary conference that meets regularly to present and discuss treatment of current cases of cancer. Since April 2010, the Tumor Board has met more than 100 times to review more than 350 cases.

Leveraging your Life Insurance Policy to Pay for LTC costs

Given the recent rate increases by many of the Long-Term Care insurers in the first part of 2016, it is easy to understand why many people today are skeptical about buying a non-guaranteed premium product.

Most people just do not want to worry about a 50% rate increase just around the corner. But not buying any coverage presents a huge problem for the entire extended family as mom & dad grow older with no real protection and the threat of catastrophic medical expenses due to chronic illness looming over their heads.

One solution to this problem is to leverage an existing life insurance policy that mom & dad currently own. Accelerating the living benefits from the existing life insurance policy while the owner is still alive can help the family to pay for long-term care expenses. Instead of surrendering the policy for cash or allowing a policy to lapse, the owner can convert the policy into a “Long-Term Care Benefit Plan”. What does converting a policy mean? It means the policy is sold as a “Life Settlement” for a percentage of the death benefit and the funds are placed in a protected Benefit Account. Monthly payments for long term care services are deducted directly from the benefit account and are tax-free.

Life insurance is legally recognized as an asset and the owner’s property rights are guaranteed by a 100 year old Supreme Court decision. The “Long-Term Care Benefit Plan” is a unique financial option for seniors because all health conditions are accepted, and there are no waiting periods, no care limitations, no costs to apply, no requirement to be terminally ill, no premium payments and no loan to pay back.

What is a Life Settlement?

An important step in converting a life insurance policy into a long-term care benefit is a transaction called a Life Settlement or Viatical Settlement. A Life or Viatical Settlement is the sale of an existing life insurance policy to a third party for more than its cash surrender value, but less than its net death benefit. In exchange for paying the purchase price to the seller as well as assuming responsibility for future premiums, the buyer will receive the full death benefit when the original insured dies.

Most states regulate life and viatical settlement transactions and many of the specific regulations in place are intended to protect individuals that are selling their insurance policies. For example, in most states, the seller of an insurance policy through a life settlement can cancel the transaction by returning the purchase price within 15 days of receipt. More than 40 states have regulations in place regarding the sale of life insurance policies to third parties.

What is the difference between a Life Settlement and a Viatical Settlement?

Often times, the terms “Life Settlement” and “Viatical Settlement” are used interchangeably. However, some state regulations use the term Viatical Settlement to refer to cases where the person covered by the insurance policy is terminally ill or has a shorter expected life span (e.g., less than two years). For those states which make the distinction, Viatical Settlement transactions may be subject to additional regulatory requirements (e.g. minimum pricing relative to the death benefit).

What is a Provider?

Life Settlement Providers are private companies that are licensed by state governments to serve as the purchaser in a life settlement transaction. Providers are responsible for ensuring that each transaction complies with applicable regulations.

The top Providers in the industry fund many transactions each year and have in-house compliance departments to carefully review transactions. The standard forms used by a Provider for transactions in a particular state are usually subject to review and approval by the regulators in that state.

Stay physically active The summer is a great time to play outdoor games, garden, or walk. Start a new routine that combines fun and physical activity. Active people are less likely than inactive people to be obese or to have high blood pressure, diabetes, osteoporosis, coronary artery disease and stroke, depression, colon cancer, premature death.

Tips Adults should get 30 minutes of moderate intensity physical activity daily.

Children and adolescents should participate in at least 60 minutes of moderateintensity physical activity daily.

Start at an easy pace and increase time and distance gradually.

Don't overdo it. Prevent injuries.

Stay cool Sun protection is important all year round, not just during the summer or at the beach. Take steps to help prevent skin cancer and other conditions. Heat-related deaths and illness are preventable, yet many people succumb to extreme heat each year.

Lower your risk for heat-related illness.

Tips When possible, avoid outdoor activities during midday, when the sun's rays are strongest.

Cover up with clothing and a wide-brimmed hat to protect exposed skin.

Drink plenty of cool, non-alcoholic fluids.Seek shade.

Wear sunscreen and lip screen with a sun protective factor (SPF) of 15 or higher.

Watch the water According to several studies, males are three times more likely than females to die from unintentional drownings in the United States. Alcohol use is involved in up to half of adolescent and adult deaths associated with water recreation.

Tips: Learn how to swim. Never swim alone.

Wear your life jacket while boating.

Avoid alcoholic beverages while boating.

Watch children in and around water.

If you have a swimming pool at your home, install a four-sided isolation pool fence.

Eat healthy Whether you plan to grill on the patio or picnic in the park, be sure to eat balanced meals with fruits and vegetables. Fruits and vegetables have important vitamins, minerals, and fiber that may protect you from some chronic diseases. Foodborne disease is caused by consuming contaminated foods or beverages. An estimated 76 million cases of foodborne disease occur each year in the United States. Most of these cases are mild and cause symptoms for a day or two, but some cases are more serious and require hospitalization.

Tips: Eat plenty fruits and vegetables daily.

Wash your hands before eating or preparing food.

Cook all foods to the proper temperatures.

Refrigerate foods promptly.

Handle and prepare food safely.

The debilitating pain and disability of migraine also attacks the emotional, social and financial fabric of a family, according to a new study conducted by researchers at Montefiore Headache Center, Albert Einstein College of Medicine, affiliated with the Uniformed Services University of the Health Sciences, Vedanta Research, the Mayo Clinic and Allergan plc. The findings were published today in Volume 91, Issue 5 of Mayo Clinic Proceedings.

The Chronic Migraine Epidemiology and Outcomes (CaMEO) Study surveyed 4,022 people with migraine as well as their spouses or domestic partners to gain a full picture of the effect of migraine on the family. Approximately 41% of people with migraine and 23% of spouses stated that they believed those impacted by migraine would be better parents if they did not have migraine, which consequently led to half of migraineurs missing at least one family activity in the past month. Approximately onethird of people with migraine and 21% of their spouses/partners endorsed worries about long-term financial security for their family due to migraine. Perceived burden was the greatest among people with chronic migraine (migraine with headache on 15 or more days per month) and among those with more frequent attacks.

“This study highlights the significant burden that migraine can have on a wide range of family activities, parenting responsibilities, spousal relationships and family finances,” said study lead author Dawn C. Buse, Ph.D., director of Behavioral Medicine, Montefiore Headache Center and associate professor of neurology at Albert Einstein College of Medicine.

“Respondents with migraine and their partners noted a great deal of emotional distress related to how this condition affects their family member including guilt, worry and sadness.

These findings underscore the challenges and negative impact that people with migraine and their family members’ experience.”

The CaMEO Study is a U.S. web-panel study that gathered information on the perceived impact of migraine—episodic or chronic-- on people with migraine, their partners and their children by surveying each of the three types of respondents from September 2012 to November 2013. As the study authors point out, an accurate diagnosis and appropriate treatment are essential to reducing the overall burden of migraine.

“The consequences of migraine can be devastating and farreaching for people with migraine and their families,” said Richard B. Lipton, M.D., director of Montefiore Headache Center and vice chair of neurology, and the Edwin S. Lowe Chair in Neurology, Albert Einstein College of Medicine. “As a next step, we are analyzing responses from the children of those with migraine, who are 13 and older. We are also developing the “Impact of Migraine on Partners and Children Scale (IMPACS)” to help quantify the family burden of migraine. We hope that improved diagnosis, treatment and enhanced recognition of the burden associated with migraine leads to better outcomes for all family members,” concluded Dr. Lipton, who is the study Primary Investigator.

Skin care: 5 tips for healthy skin

Good skin care — including sun protection and gentle cleansing — can keep your skin healthy and glowing for years to come.

Don’t have time for intensive skin care? You can still pamper yourself by acing the basics. Good skin care and healthy lifestyle choices can help delay the natural aging process and prevent various skin problems. Get started with these five nononsense tips.

1. Protect yourself from the sun One of the most important ways to take care of your skin is to protect it from the sun. A lifetime of sun exposure can cause wrinkles, age spots and other skin problems — as well as increase the risk of skin cancer.

For the most complete sun protection: • Use sunscreen. Use a broad-spectrum sunscreen with an SPF of at least 15. Apply sunscreen generously, and reapply every two hours — or more often if you're swimming or perspiring.

• Seek shade. Avoid the sun between 10 a.m. and 2 p.m., when the sun's rays are strongest.

• Wear protective clothing. Cover your skin with longsleeved shirts, long pants and wide-brimmed hats.

2. Don't smoke Smoking makes your skin look older and contributes to wrinkles. Smoking narrows the tiny blood vessels in the outermost layers of skin, which decreases blood flow. This depletes the skin of oxygen and nutrients that are important to skin health.

3. Treat your skin gently Daily cleansing and shaving can take a toll on your skin.

• Limit bath time. Hot water and long showers or baths remove oils from your skin. Limit your bath or shower time, and use warm — rather than hot — water.

• Avoid strong soaps. Strong soaps and detergents can strip oil from your skin. Instead, choose mild cleansers.

• Shave carefully. To protect and lubricate your skin, apply shaving cream, lotion or gel before shaving. Shave in the direction the hair grows, not against it.

• Pat dry. After washing or bathing, gently pat or blot your skin dry with a towel so that some moisture remains on your skin. If your skin is dry, use a moisturizer that fits your skin type.

4. Eat a healthy diet A healthy diet can help you look and feel your best.

Eat plenty of fruits, vegetables, whole grains and lean proteins. The association between diet and acne isn't clear — but some research suggests that a diet rich in vitamin C and low in unhealthy fats and processed or refined carbohydrates might promote younger looking skin.

5. Manage stress Uncontrolled stress can make your skin more sensitive and trigger acne breakouts and other skin problems. To encourage healthy skin — and a healthy state of mind — take steps to manage your stress.

Recent advances in Cosmetic Surgery

One of the most exciting, recent advances in cosmetic surgery has been the use of the endoscope infacial rejuvenation.

An endoscope is a small camera which can be inserted through a tiny cut in the skin and which transmits an image of the underlying tissues to a T.V. Screen. Many people are familiar with endoscopy as it is applied by orthopedic surgeons to treat cartilage damage in joints (arthroscopy) and by gynecologists to examine pelvic organs (laparoscopy). This relatively old technology has now been applied to cosmetic surgery. Plastic surgeons are now able to perform brow lifts through several tiny incisions in the scalp. Until recently, the only way a brow lift could be performed was with an “ear to ear” incision.

Even more exciting is that plastic surgeons are now performing face lifts through the endoscope. In those patients whose complaints are of mid face and jowl sagging rather than skin excess, a face lift can be performed through several small incisions in the scalp and lower eyelids. The best candidates for this procedure are those patients in their 30s and 40s with good skin tone. Recovery time is somewhat longer than with traditional facelifts due to swelling that persists for several weeks. However, these procedures have allowed the treatment of the aging face to extend to much younger patients with improved long term results.

Just as the endoscope has revolutionized facial rejuvenation, so too has the tumescent technique of liposuction changed how plastic surgeons reduce adipose fat tissue in all areas of the body.

With tumescent liposuction, a surgeon distends the fat and soft tissues with local anesthesia, adrenaline, and salt water. By doing so, blood loss has been shown to be dramatically reduced as compared to more traditional technique of liposuction, far more fat can be safely aspirated at one sitting, frequently without the necessity of hospitalization or even general anesthesia. Currently, experimental procedures involving ultrasound and liposuction are being perfected which will hopefully improve techniques further. Regardless of the technique, liposuction is still not a treatment for generalized obesity, but rather for treatment of localized fat excess and disproportion.

widespread media coverage, lasers are now thought of by many lay people as the best way for surgeons to cut tissue during surgery. While lasers represent new and exciting technology, this notion is an unfortunate misconception. For most operations, traditional cutting methods provide better control for the surgeon and more precise and smaller scars. Where, then, do lasers play a role?

One of the new and interesting uses of lasers is in skin resurfacing. Specifically, carbon dioxide lasers, pulsed at very high frequencies are now being used by some plastic surgeons instead of dermabrasion. By using such a laser, fine wrinkles around the mouth and eyelids can be markedly reduced with reportedly less tissue destruction than seen with traditional dermabrasion techniques.

Removal of tattoos is another use of lasers, Different laser types are needed for various colors found in the average tattoo.

Several surgical settings are frequently needed to eliminate a tattoo completely.

Scarring tends to be less than with either direct surgical excision or dermabrasion.

Lasers can also be used to treat red, vascular lesions such as port wine stains. Lifelong lesions can be dramatically reduced using laser surgery in a painless, outpatient setting.

Finally, lasers can be utilized to reduce age spots on hands and other locations on the body, so that a newly “lifted face” can have youthful hands to match.

Who says that youth has to be wasted on the young?

Along with advances in cosmetic surgical techniques, have come improvements in plastic surgeons' abilities to communicate with their patients. One tool for aiding in such communication has been new computer hardware and software that allows a physician to photograph a patient and modify their image on the computer screen. By doing so, a surgeon can show a patient an image of a reasonable outcome of a cosmetic procedure. It also gives a patient the opportunity to show a doctor what they like and dislike about their current features or about the proposed outcome of a cosmetic procedure. Good physician-patient communication is essential to maintaining a satisfied patient. Plenty of people feel anxious about the fact that they overeat. And for decades, Julie Latz was one of them.

“I spent 45 years being obsessed with food,” she says. “I would gain weight, try every diet that was out there, and go through the yo-yo cycle of losing weight, but then binge and regain it. Sometimes I’d gain as much as 40 to 50 pounds, and then go back on a drastic diet.”

Finally, one day about six years ago, Latz says, she had enough. “I said to myself, ‘I’m not going to live like this anymore!’”

Tired of the shame and guilt around her yo-yo dieting and food obsession, she created a simple program to help herself trim down, but more importantly, to stop the madness of food addiction – and it worked.

“I realized that going from diet to diet was a dead end which never has lasting results,” she says. “Dieting leads to deprivation, and deprivation leads back to bingeing.”

The program helped her reach what she calls “the magic zone.” She explains: “It’s a very calm place where there’s no feeling of deprivation and no urge to binge.”

Latz, based in New Rochelle, is a certified Food Psychology Coach through the Spencer Institute. For 4½ years, she’s been offering her Peaceful Eater program to oth-

ers - helping men, women and teens all over the world break free from the dieting/binge-eating cycle that is ruining their lives.

“Once they stop bingeing, weight loss follows,” she says. “And as they become calm around food and learn how to eat whatever they’d like in moderation, they enjoy making conscious choices of how to nourish themselves. They feel in control of what they put in their bodies instead of feeling controlled by the food.”

Baby weighed just 9 ounces at birth; one of the lightest to survive extreme prematurity in New York State

Maria Fareri Children’s Hospital held a birthday party recently for a very special girl. Born at only 25 weeks last April, Tamaya Berry weighed just 9 ounces when she arrived at the Regional Neonatal Intensive Care Unit (NICU) at Maria Fareri Children’s Hospital, a Member of the Westchester Medical Center Health Network and the children’s hospital for the Hudson Valley and Fairfield County. Tamaya spent six months at the hospital and after overcoming many challenges due to her low birthweight and extreme prematurity, Tamaya graduated from the Regional NICU in October 2015 weighing 7 pounds, 12 ounces. To mark Tamaya’s milestone first birthday and to celebrate her perseverance, the staff of Maria Fareri Children’s Hospital’s Regional NICU hosted a birthday party for Tamaya and her family.

What do doctors do to treat their own pain?

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result.

Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead.

My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked.

If it hadn't happened to me, I wouldn't have believed it. During my last flare up, I was expecting to be on my back for 2-3 days and in pain for 2 weeks. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life.
