Initial As PARENT or GUARDIAN and PLAYER. I WILL (please initial next to each item): Be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other AVA event. Parents need to remain in the spectator areas during all games and practices. Respect the officials and their authority during games. Refrain from yelling at the linesmen, scorekeepers and officials. Not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures. Wait 24 hours before contact with any AVA staff. I will follow the club grievance procedures as outlined in the AVA Parent / Player handbook. Refrain from coaching my child or other players during games and practices. Encourage my child to discuss issues with the coach. Show my support for the entire team and not just my child. Teach my child that doing one’s best is more important than winning so my child will never feel defeated by the outcome of a game or her performance. Make sure my child refrains from drugs, tobacco and alcohol during any AVA sponsored event. Inform the coach of any conflicts with practices and tournaments well in advance of the event so the coach can adequately prepare and make adjustments. The only excuse for missing a practice is illness or a death in the family. If a player is to miss a practice, she must inform her coach 24 hours in advance. If there is a conflict between a player’s commitment to AVA of Texas and a school activity or college visitation, the athlete must give the coach advanced notice of her absence. Homework and tests are not an excuse for missing practice. Practice schedules are made well in advance in order that the student athlete can manage her time accordingly. Missed practices are to be made up as soon as possible by attending another team’s practice. You are to notify your coach when you will be attending so your coach can let the coach of that practice be aware of your participation. There will be a limit of 3 makeup practices. Consistent tardiness and missed practices will result in less playing time and possible removal from the team. I understand that AVA does not guarantee play time and I will not cause stress to the team by any negative behavior displaying my disappointment in lack of play time. I understand that AVA has the right to move players to different teams throughout the season and that coaching assignments may change. Inform the coach of any physical disability or ailment that may affect the safety of my child in advance. If an injury occurs during practice or tournaments, the player will notify her coach immediately. As stated in the handbook, players are required to seek medical attention immediately to asses and determine health/playing status. Injured players are expected to attend all practices and local tournaments. Parents are not allowed to move practices or any AVA scheduled function at any time. This decision will be made by the directors and can only be handled through the directors and the appropriate coach. No parents or players are to address other clubs organizations concerning tournament or bracket play if you are dissatisfied with pool seedings, playoff formats, or general tournament procedures. Please address AVA directors and staff to voice your concerns. The directors will evaluate the situation and correspond with the other club directors if needed. Participate in all tournaments on the schedule including post-season play. It is mandatory that all players attend National qualifiers, regional qualifying tournaments and National Championship Tournaments. Players selected for a travel team must participate in all tournaments. We reserve the right to move players to a local team if they do not participate in travel tournaments.

Please do not book your reservations for out of town and out of state tournaments until the directors and travel coordinator have given you the final confirmation. AVA will NOT be responsible for loss of travel expenses due to early booking. Issued equipment (including uniforms) is property of AVA until the end of the season. AVA reserves the rights to switch uniforms as needed. Participate in all facility duties assigned to my team before and after practice. Work at least one assigned AVA host tournament with my team. Work at least one assigned AVA-tor Saturday event with my team. Follow all the rules of the practice and tournament facilities such as NO JEWELRY, NO FOOD, NO COOLERS, NO SMOKING, NO GUM!!!! throwing away trash in proper receptacles, etc. Ensure that relatives and guests abide by the AVA Parent / Player Handbook. AVA does not guarantee college scholarships. However, AVA does provide recruiting services for players in the high school age divisions. If you chose not to follow the steps required to get recruited, AVA reserves the right to stop recruiting services for you. Agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: verbal warning by official, head coach, tournament director and/or AVA Directors. Written warning / reprimand. Parent or player game / match / tournament suspension with written documentation of incident kept on file by organizations involved. Player and / or Parental season suspension. I have read the Fee Schedule for the 2013-2014 season and agree to pay the fees for my daughter’s participation in full. I also agree to participate in the one mandatory club fundraiser at the beginning of the season and to fulfill the minimum required obligation before the practice season begins in November. I am aware of my financial obligations as a parent/guardian, and understand that all payments must be made ON or BEFORE the due date. I understand that if I do not meet payment deadlines, my daughter will not be allowed to participate in practice or tournaments until all payments are current. I understand that all payments made to AVA of Texas are non-refundable except in the case of season ending injury that causes a player to have to withdraw from the program during the 2013 calendar year with proper documentation from the attending physician. Any injury sustained in the 2014 calendar year will not be refunded, but certain extenuating circumstances may be brought to the board of directors for an executive decision. I have given permission for my daughter to play for AVA of Texas and I fully understand the commitment involved. I agree to abide by the AVA Parent / Player Handbook. Allow AVA to use my daughter’s photos either in print or internet.
