Aftercare Recovery is a lifelong commitment and lifestyle. Clients meet with their primary therapist and/or case manager to discuss ongoing aftercare plans, and when appropriate
Referral sources, FMLA paperwork, connections to sober living, follow-ups to outpatient care, individual therapy sessions, reintegration back into the family and any other needs are assessed and scheduled by the facility staff.
The final plan provides clients with the appropriate contacts, resources and referrals allowing recovery to continue. Immediately following discharge we provide medication information, recovery maintenance services for followups and specific discharge plans to further ensure sobriety.
