EHR (Electronic Health Record) Project by AksaTech Solutions pvt ltd SEARCH FOR PATIENT: In this option, we can search patient within the clinic. For search we can click the check box all patients to get all their details or patient id or other unique info regarding to patients so that we can pull up the needed data. When we click the search button we will be getting the needed patients name and their respective data and when we click on the patient then it goes to the dashboard for that Patient. We can activate and inactivate a particular patient by clicking the Active or Inactive link under the Action. Schedule Day Sheet: Here we can get a report of all scheduled patient’s for the current day with an order by Patient name, Chronological order and chart number as selected.

ADD NEW PATIENT: This is the next option with the help of which a new patient can be created. This page starts with a heading called new demographics. The basic information which has to be added is like: First name, MRN no, DOB, last name, languages known etc. Next is the Health Card which has to be filled with the details like: Number, Version code, Provience and Expiry Date. After this the patient has to fill up information regarding      

Patient Contact Information: Patient Employer/School Details Patient Pharmacy Alternate Contact Patient Choice Upload photo of the patient Fields with “*” are mandatory.

PROFILE: This is the next option with the help of which the details regarding the provider can be updated if logged in by a provider or the details regarding the other User can be updated if logged in by other user. 1)

Provider Login:

For Example: The details regarding this are: First Name, Last Name, Physician ID, phone1, phone2 and email id etc.

The schedule table below shows the days and the corresponding sessions with which the definite timings of the provider is added and by clicking the save changes button the changes which have been made gets updated. 2)User Login: The profile of the user will be having details like: First Name, Last Name, Physician ID, phone1, phone2 and email id etc. to be updated. Preferred Drug List: Here with the help of the auto-completer, drug name can be entered and its corresponding details will be entered automatically. All these details would be added into the database by the provider to create his/her favorite drug list for frequent use, for the purpose of creating a prescription for a patient.

PATIENT SPECIFIC: Once we get the search result from the detailed search, we can pull a patient record by selecting a patient. After a patient is selected, we will get the dash board for the selected patient. The dashboard contains brief information for the selected patient. The dashboard contains a brief introduction for demographics, History, Encounter and Lab information. By clicking the report under History Report all history information for the patient can be viewed in a report format. Similarly, by clicking the report under Allergy Report and Immunization Report all allergy information and immunization information for the patient can be viewed in a report format. By clicking the date under Date of Service, the corresponding encounter page will get open. By clicking the report under Encounter Report, Summary Report and Order Report, the encounter information, the patient summary information and patient order information for that particular date of service can be viewed in a report format.

Encounter: If we click on the encounters button at the top a new encounter page will get opened. Here we will get the following fields; 

General: It contains general information such as date, type of encounter, referred by and attended by. We have to select this by drag down button as shown in snapshot. Date refers to the date of service; it will automatically come based on the current date.

Chief Complaints: These fields should be filled if the patient is having any chief complaints. After a user start typing letters inside the above textbox under chief complaints, auto completer will give suggestion based on data in the database. After an entry is selected pressing of Enter button of keyboard will make an entry of a chief complaints. In order to remove the entry clicking on the “X” button will remove it. Clicking on the “X” button will remove the last entry. If there are two or more entry is made and user want to delete an entry apart from last entry then that entry needs to be made in the above text box and then click off the “X” button will remove it. User cannot make a duplicate entry.

Vital Signs: It consists of fields such as BP, TP, P, R, Wt, Ht and BMI. Fields like TP, Wt and Ht have two units. Making an entry in one unit will automatically makes an entry to the other; however the other entry should be kept blank. BMI entry will come automatically based on Ht and Wt entry. If a patient is having other vitals details apart from the above mentioned entries then those entries can be made by clicking of the “+” button, and deleting of those entries can be made via the “X ” button.

Changes from Last Visit: It will give graphical representation of standard vital signs i.e. BP, TP, P, R, Wt, Ht and BMI over the different date of services that the patient has gone through. User can also makes a note based on those graph in the below note textbox.

Review of Systems: These fields should be checked after the patient has been examined. For example: if patient is having eye problem eye checkbox should be checked. If any additional entry needs to be made then those entries at first needs to be made in master data under admin panel.

Reviews: These fields should be checked based on the assessment. If the patient history data needs to be checked then clicking on the view History will show up those details.

Diagnosis: Individual diagnosis entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in description auto completer will suggest based on database entries. After diagnosis is entered the corresponding code will come automatically and vice versa. A user cannot make an entry in diagnosis and code apart the database entries those are shown by suggestions.

Lab Orders: Individual lab order entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in Test Name auto completer will suggest based on previous entries. A user cannot make an entry in Test Name apart the database entries those are shown by suggestions. All entered diagnosis entry will come as check boxes for individual orders, user can checked the required diagnosis to make note the for what diagnosis the order has been made. If any diagnosis is entered after the order entry then those diagnosis will come as checkboxes inside the already entered Order entry by clicking of the refresh button.

E-Orders: Upon clicking of this button, already entered orders for the current encounter will come in a pop-up window, where from printing can be made.

Medication: Individual medication entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in Medication Name auto completer will suggest based on previous entries. Same thing holds good for dosage, strength, frequency and form entries also. A user cannot make an entry in medication name apart the database entries those are shown by suggestions. All entered diagnosis entry will come as check boxes for individual medicines; user can check the required diagnosis to make a note, for what diagnosis the medication has been prescribed. If any diagnosis is entered after the order entry then those diagnosis will come as checkboxes inside the already entered Medicine entry by clicking of the refresh button. Upon clicking of the favorite drug button user can get a list of his/her favorite drugs and can select the appropriate drugs from that list for the prescription.

Herbal /Nutritional Supplements: Individual herbal /nutritional supplements entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in name auto completer will suggest based on previous entries. Same thing holds good for dosage, strength, frequency and form entries also.

Immunization: Individual immunization entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in immunization, auto completer will suggest based on previous entries. A user cannot make an entry in immunization apart from the database entries those are shown by suggestions. Manufacturer of the immunization will come automatically after an immunization entry will be made based on the database entries.

E-Prescription: Upon clicking of this button, already entered medication for the current encounter will come in a popup window in the form of a prescription, where from printing can be made.

CPT code: Individual CPT Code entries can be made by clicking of the “+” button. Deleting of an entry can be made by clicking of the corresponding “X” button. A user after started typing letters in Description and Code, auto completer will suggest based on previous entries. A user cannot make an entry in Description and Code apart from the database entries those are shown by suggestions. Code entry will come automatically after a description entry will be made based on the database entries and vice versa.

Patient Family Instructed on: The user will check the corresponding checkboxes based on the diagnosis. If any other entries need to be made apart from the already entered entries then the same can be made in a textbox named other.

Follow Up: The user will make an entry for follow up or join work school based on the diagnosis. This also has the auto complete feature.

Refer Patient: The user will use it to make an entry for referring the patient to other provider.

History of Present Illness: Under history of present illness there were nine entries which are description, location, quality, severity, duration, onset-timing, context, modifying factors and signs-symptoms. Apart from description all other field entries can be made same as of chief complaints. Description field entries can be made by simply typing inside the description textbox.

Physical Examination: These fields should be checked after the patient has been examined. For example: if patient is having Eyes problem Eyes checkbox should be checked. It has three levels of checkboxes for more detailed entries. Notes option is also there for each level to make a note. If any additional entry needs to be made then those entries at first needs to be made in master data under admin panel.

Service Wellness Guidelines: Here service wellness guide lines for the patient can be entered. By clicking of the get button, the service wellness guidelines with respect to the entered CPT code will be fetched from the database and fill the same.

Patient’s Preference: Here patient’s preference against the service wellness guidelines will be entered.

Patient Education: Here User will make an entry based on the observation.

Patient Specific Alert: Here alerts for the patient will be added based on the diagnosis for future reference.

Signature: This will keep track of the user who has made modification to the encounter screen. The current user can make the signature checkbox as ticked to mark his/her signature and also make a note. Making the document status as inactive will freeze the encounter.

Patient Documents: Here patient related documents can be uploaded upon clicking of the upload document button.

Document List: Here all uploaded document for the patient as well as the Alert for the patient can be viewed. Those alerts and documents can also be deleted. In order to view uploaded documents the documents at first needs to be downloaded and saved it to the disk.

Appointment History: It consists of the list of all appointment history for the patient, including past and future.
