HEALTH SERVICES CHILD HEALTH PLUS FOR WORKING FAMILIES

Child Health Plus is a state-subsidized health insurance program for children under the age of 19 who currently have limited or no health insurance. Child Health Plus is for families who do not have access to comparable, comprehensive health coverage, the child must be a New York State resident, under the age of 19 and not to be eligible for Medicaid. Benefits under Child Health Plus include: well-child care, immunizations, emergency care, hospitalization, prescription and non-prescription drugs, vision, dental, speech and hearing services and much more. There are currently no co-payment for services. The three insurance companies who administer Child Health Plus benefits in Oneida and Madison county are: HMO Blue, Kaiser Permanente, and United Health Care. The only differences in the three plans are the providers who participate with each plan and full premium cost. The Mohawk Valley Prenatal Network has been sub-contracted by the New York State HMO Council to provide outreach and education for Child Health Plus Insurance for kids in Oneida County and Family Ties Network in Madison County. EMERGENCY AUTHORIZATION FORMS

Pre-k and kindergarten students are required to see their own Health Care Provider for a physical and booster immunizations prior to the first day of school. The completed physical exam form and updated immunization certificate must be on file in the Health Office prior to the first day of school. Blood Lead Test requirement for Pre-k students: NYS Public Health Law Article 13, Title 10, Section 1370 states that Parents/Guardians must provide proof that their child has had a blood lead test within three months of enrollment.

MEDICATION DURING THE SCHOOL DAY

School policies regarding administration of medication are consistent with procedures established by Educational Law. For students to receive medication (prescription or non-prescription including cough drops and other “over the counter” drugs) during the school day, the following procedures MUST be followed and renewed each year. x x

The parent or legal guardian must submit a written request to the school authorities together with a written request from the prescribing physician indicating the frequency and dosage. The medication must be brought to the school by a responsible adult in the original container labeled with the name of the drug and dosage. Medication should NOT be brought to and from school. Please supply only what is needed during the hours of school and keep the rest at home. Students may NOT carry the medication on themselves.

These forms are important when parents are not available and it is imperative that someone responsible for the child is located. The responsibility for transporting a child in the even of illness or injury lies with the parent or guardian.

The Health History form must be filled out and signed by the student’s parent/guardian. Your signature authorizes the school authorities to seek emergency medical care for your child, and to share medical information with appropriate staff members on a need-to-know basis.

Unless all procedures are followed, medication CANNOT be administered at school.

If the above procedures are not followed discipline procedures, as outlined in the school handbook, will be followed.

STUDENT PHYSICALS

The Education Law of New York State requires that each new entrant to the district and students in grades Pre-k , K, 2nd, 4th, 7th, and 10th, must have a physical examination, either by the school doctor or family physician. A vision, hearing, growth and scoliosis screening will be done by the nurse. Parents will receive packets when school starts, or when a student enters the District. This packet includes: permission slip for a physical, health history form, physical exam form, and medication policy. Students can have a physical performed by their own

school are performed by the School Nurse Practitioner. Please read, sign and send the permission slips in as soon as possible. Students will be scheduled for a physical in school per NYS regulation if the permission or completed physical is not received in a timely manner.

Height and weight are done with health appraisals and yearly in the elementary school.
