Your role in your care You are an important member of your health care team Let us know what is important to you and how you want to be involved in your care. We respect and support your right to make informed choices about your care. We work with you to set goals, make decisions and plan treatments that meet your needs and preferences.
Tell your health care team your goals and values We want to respect your goals and values. Telling us what they are is the best way to make sure this happens. It is important to speak up if you feel your goals and values are not being respected.
Let us know how we’re doing Whether you have questions, a complaint, a suggestion or a compliment, UHN Patient Relations would like to hear your feedback. We can speak with any patient, family member or visitor of UHN. Find more information on Patient Relations on page 43.
