ADULT DAY CARE CENTERS Adult day care is a planned program of activities designed to promote well-being though social and health related services. Adult day care centers operate during daytime hours, Monday through Friday, providing a safe, supportive and cheerful environment. Nutritious meals that accommodate special diets are typically included, along with an afternoon snack. Adult day care centers can be public or private, nonprofit or for-profit. The intent of an adult day center is twofold: • To provide older adults an opportunity to get out of the house and receive both mental and social stimulation • To give caregivers a much-needed break to attend to personal needs, or simply rest and relax Adult day care centers are licensed, monitored group facilities that provide various services including medication management, various types of therapies and meals.

KINSHIP CARE PROGRAM Kinship care is when grandparents or other close relatives provide fulltime child care in their homes for another relative’s child or children. The primary reason for providing this support is because the parents of the children are absent or unable to properly care for them. kinship care is the fastest growing type of placement method for children in the country. Families and social service agencies realize that children greatly benefit when families are kept together. Living with a responsible adult relative sends a positive message to children that they still belong and are loved. It also helps them believe it is not their fault their parents are not raising them. As a result, they grow up feeling stable and secure. The need for kinship care can come about in a number of ways. • A grandparent observes that grandchildren are always dirty, hungry and maybe even bruised. They may even suspect child abuse. They call Child Protective Services for assistance to initiate the removal of abused children from the home, or the offer to take care of them in their own homes. • A relative dies, leaving children orphaned. As the closest relative, you consider caring for these children in your home.

Kinship care is provided when close relatives or friends provide child care support in their homes, raising children of parents who are absent or unable to care for them.

SEAN MURPHY/DIGITAL VISION/THINKSTOCK

• A mother or father is contacted by an attorney, courts or social services regarding the incarceration of their child who has children. The grandparents or close relatives do not want the children going into foster care. • Drug and alcohol addiction prevents a relative from responsibility caring for their own child or children, and they abandon them, leaving them with you for weeks. If you are a grandparent 55 or older and you have the primary responsibility for the care of minor children, the Prince George’s County Administration on Aging wants you to know you don’t have to handle this alone. Call us for resource information at 301265-8450.

hether you’re settling into your 60s or heading into your ninth decade, you should be extra careful when taking prescription and over-the-counter medicines. And if you’re caring for older loved ones, you should help them stay safe. The older you get, the more likely you are to use additional medicines, which can increase the chance of harmful drug interactions. And, as we age, body changes can affect the way medicines are absorbed, leading to potential complications. For instance, your liver and kidneys might not

work as well, which affects how a drug breaks down and leaves your body. And changes in your digestive system can affect how fast drugs get to your bloodstream. “There is no question that physiology changes as we age. Many chronic medical conditions don’t even appear until our later years,” explained Sandra L. Kweder, M.D., deputy director of the Food and Drug Administration’s Office of New Drugs. “It’s not that people are falling to pieces; some changes are just part of the normal aging process.”

TAKE MEDICINE AS PRESCRIBED Take your medicine regularly and according to your health care provider’s instructions. Don’t skip doses or stop taking medication without first consulting with your provider. This holds true even if you’re feeling better or if you think the medicine isn’t working. “Medication can’t work unless you take it,” Kweder said. “For instance, medications that treat chronic conditions such as high blood pressure and diabetes typically only work when taken regularly and as directed. You have to take them continuously to maintain control over your condition.”

20 | 2014-2015 SENIOR RESOURCE GUIDE SPECIAL ADVERTISING SUPPLEMENT TO THE GAZETTE & THE GAZETTE-STAR

Dosing for medications is based on clinical trials, which the FDA reviews. “Every medicine is really different and is dosed according to what’s been tested,” said Kweder, which is one reason why you shouldn’t select a dose yourself. If you’re having bothersome side effects or have other questions, talk to your health care provider. KEEP A MEDICATION LIST Write down what you’re taking and keep the list with you. Consider giving a copy to a friend or loved one whom you trust—an important step especially in case of emergency and when you’re traveling. Record the medicine’s brand name, if applicable, and generic name. Also write down how often and what dosage you take. (For instance, one pill daily, 300 milligrams.) Finally, note when you take each drug. “You should know your medicines better than the doctor does,” said Kweder. BE AWARE OF POTENTIAL INTERACTIONS Remember, as you age, you’re at higher risk for drug interactions. Interactions can occur when: • One drug affects how another drug works; • A medical condition you have makes a certain drug potentially harmful;

“There is no question that physiology changes as we age. Many chronic medical conditions don’t even appear until our later years” - Sandra L. Kweder, M.D., deputy director of the Food and Drug Administration’s Office of New Drugs.

• A food or non-alcoholic drink reacts with a drug; • A medicine interacts with an alcoholic drink. Your task? Learn which interactions are possible. You can do this by carefully reading drug facts labels on over-the-counter drugs and the information that comes with your prescription medications, and by reviewing any special instructions with your health care provider. For instance, nitroglycerin, which treats angina (chest pain related to heart disease), should not be taken with many erectile dysfunction drugs, including Viagra and Cialis, because serious interactions can occur. And some drugs should not be taken with alcohol, as symptoms such as loss of coordination and memory loss can result. If you’re seeing multiple health care providers, tell each one about all of your medications and supple-

ments. You also can ask your pharmacist about potential interactions. REVIEW MEDICATIONS WITH YOUR HEALTH CARE PROVIDER Schedule at least one annual review of your medications with your health care provider to confirm which medications are still necessary and which, if any, you can stop taking. If a certain medication seems out of your budget, ask your health care provider whether there is a cheaper, and still effective, alternative. This review can help you avoid interactions and can lessen costs. Sometimes, especially if you’re seeing multiple providers, certain questions can fall through the cracks, said Kweder. But, she said, there is no such thing as a stupid question about medicine. “As a society, we have become reliant on pharmaceuticals to help us attain a longer and higher-quality life. It’s a wonderful success of Western medicine,” she said. “The goal should be for each of us to access that benefit but respect that medicines are serious business. To get the most out of them, you should take them with great care and according to directions.” -U.S. Food and Drug Administration

Research has shown that 50 percent of older adults who live on their own and 75 to 85 percent of seniors in care facilities suffer from chronic pain.

C

hronic pain is a growing problem among older Americans. Understanding the causes of this pain, the special medical needs of the elderly, and the role of pain self-management can help seniors reduce or eliminate this condition. As the U.S. evolves into a nation with an older population, greater attention is being paid to health care problems more common among seniors. Research has shown that 50 percent of older adults who live on their own and 75 to 85 percent of seniors in care facilities suffer from chronic pain. Yet, pain among older adults is largely undertreated and can have serious consequences, such as depression, anxiety, decreased mobility, social isolation and poor sleep. There are natural changes that occur with aging that affect pain—sleep patterns change, muscles and joints gradually become more rigid, and energy decreases. Frequently, older people don’t report their pain because they don’t know that it can be treated, or they believe it will lead to expensive tests or more medications. And there can be conditions, such as vision or hearing loss or dementia, that can limit communication about pain. To help overcome these barriers, treatment of chronic pain may involve a team of different pain management specialists— including a physician, nurse practitioner, physician assistant, pharmacist and others who specialize in pain management. Treating pain in older adults requires special care because nutritional problems or multiple medical problems—diabetes, heart disease, arthritis—are common. Helping seniors self-manage their pain is an important part of reducing or eliminating that pain.

QUESTIONS TO ASK YOUR HEALTH CARE PROVIDER Communication with your caregiver or care team is the best way to help you manage or end your chronic pain. Ask these questions: • What is causing my pain? What can I do about it? • What is the name of the pain medicine I will be taking? • How long will it take for the medicine to work? • What side effects should I expect? • If I forget to take the pain medicine, what should I do? • When should I take the pain medicine—on a regular schedule? Before, with, or after meals? At bedtime? • Are there any dangers to taking this pain medicine I should know about? • Will this pain medicine cause problems with any other prescription drugs or over-the-counter medicines I am taking?
