How I love summertime. I look forward to it all year. Spending time with the kids, long walks on the beach, bike riding, playing with the dog, lazy days beside the pool. Travelling is fantastic, but there really is nothing like a holiday in your own backyard. And it’s a great time to take stock; look at the year that has just passed, and make plans for the one about to come. When I was younger I would make wild resolutions that I had zero chance of achieving. These days, my goals seem far simpler, but sometimes just as unachievable. I’m just striving to be as healthy and happy as I can, the best version of myself, which is not always easy, especially at this time of year. If like me, you need a little help staying on track with your health and fitness goals, SLIM is the magazine for you. Once again this issue we have so many informative and entertaining stories. Our cover girl Gwyneth Paltrow is one celebrity who divides opinions, but there is no denying she exudes good health. We look at her life and loves, and her new role as the face of Hugo Boss. A new study suggests that those who follow a vegetarian diet are healthier and may even live longer than carnivores. As part of a study by Loma Linda University in California, more than 73,000 participants were asked about their eating habits and were separated into categories based on how often they ate dairy, eggs, fish and meat. Over a six year period, researchers noted that seven people died of any cause per 1,000 meat eaters each year compared to about five or six deaths per 1,000 vegetarians. This means there was a 12% lower risk of dying from any cause for vegetarians compared with nonvegetarians. The study also found that vegetarians tended to drink less alcohol, smoke less, exercise more and to be thinner. “I think this adds to the evidence showing the possible beneficial effect of vegetarian diets in the prevention of chronic diseases and the improvement of longevity,” said lead researcher Dr. Michael Orlich. He believes the benefits come from lower blood pressure and improved cholesterol levels in people eating diets based on vegetables, whole grains and fruit. According to the National Health and Medical Research Council (NHMRC), Australians need to eat more vegetables, legumes, fruit and fish and decrease their red meat intake by 20%. They suggest no more than 500g of red meat per week.

Cut the Turkey Breast Steaks into large cubes. Heat the oil in a large heavy based pan over a medium heat. Sear the turkey for 5 minutes or until golden. Remove and set aside. Add the spring onions, garlic and Thai red curry paste to the pan and cook for 1 minute. Pour over the coconut milk and stock. Then add the fish sauce and sugar with the lime leaves. Stir until simmering. Add the pumpkin, reduce heat to low. Cover and cook for 10 minutes or until pumpkin is tender. Return the turkey to the pan with the beans and simmer, uncovered for 5 minutes. Sisters, we’re doing it to ourselves

W

e can be our own worst body critics, and when confronted with a picture of ourselves, most of us will see a negative image, irrespective of what the reality is. The clinical diagnosis used to explain this distortion of reality is called Body Dysmorphic Disorder. This disorder often happens together with an eating disorder such as Anorexia Nervosa and Bulimia. These mental health difficulties are borne from anxieties that have been given opportunity to manifest in body image. The management of body anxiety can become engrained and entrenched patterns of behaviour through daily rituals. One must question: are your thoughts and behaviours toxic or value adding, because ultimately what we practice, we eventually become. Many may admit to having destroyed or deleted photos of themselves they are not happy with, and some people do not have photos of a once in a lifetime event. I have even consulted with women who do not own photos of themselves on their wedding day, due to their unhappiness with their looks. Most women I have consulted have untagged themselves on social media and almost all have enhanced photos of themselves before uploading the photo to social media as a way to ease their anxiety about their body image. If the thought of having a photo taken of yourself makes you more anxious than public speaking, you are not alone, as most people I consult with would find it to be one of the more confronting and anxiety-provoking social norms of the digital age. With the uploading of so many photo galleries onto social media websites and tagging of photos to for everyone to see, we can understand the rising incidence of body anxiety, which has climbed steadily in direct correlation with the limitless bounds of social media. This raises a question for us to challenge ourselves with “Why did you stop thinking you were beautiful?” To answer this question, we 24 slim magazine summer 2014

need to try to recapture the feeling we had when we were young and fearless in front of the camera; when we embraced our own beauty, without reservation. The answer can also be found by looking at which beauty morals and ethics you have passed onto other woman, especially those younger and more impressionable. It is well reported that girls mirror the behaviour of their mothers about body image, beauty, confidence and self-esteem. So mothers and other female role models need to realise their own beauty and set positive examples for their daughters or younger family members. In explaining the body anxiety theory, women’s responses and reactions to other women highlight how women are pre-programmed to be more critical and evaluate other women negatively. Countless psychological/sociological research papers have detailed how women will envy the body image of a thinner body type, which they think is more desirable. This body type might well be more appealing for other women, however, men consistently value a female body type which has a higher BMI and is shapelier to be more attractive, compared to the thinner body type that women find more attractive. Likewise, men will consistently say they find a more natural-looking face more attractive, with softer bone structure – mirroring the softer


If a woman acts differently, in a way not expected, other women will turn on her. If they can destroy the rebel’s self-confidence they can make her change and conform to the remainder of the ‘in group’.

Also reroute your focus to concentrating on your body’s strengths and less time on your physicality’s weakness. P sychology tip: Use positive affirmations and mantras to shape your internal dialogue, also verbally congratulate others when you notice something inspiring about their body image. F ace your body anxiety fears one at a time, slowly and

An important footnote to these studies of female attractiveness is that the more natural female faces desired by men often had minimal makeup. In short, women might consider trying less hard to please other women, and in doing so find they are more attractive to the opposite sex (and happier to boot!). American neuro-psychiatrist Ms Louann Brizendine theorises about the way women psychologically bind to one another in her book The Female Brain. In her book she explains that it’s programmed into women to connect with other women intensely in their childbearing years, because we need the support and protection of other women when the males are away (e.g. hunting by our forefathers and at the office in the modern age). This need for female bonding can been seen in the way women control one another, namely the idea that “if everyone acts and thinks the same way, the group will stay intact, everyone will survive and we will stay protected”. Put simply, there is a need to control females in the group as a matter of survival. Obviously, as a human species we have evolved and progressed, however this hypothesis by Ms Brizendine suggests that as a function of our social programming, if a woman acts differently, in a way not expected, other women will turn on her. If they can destroy the rebel’s self-confidence they can make her change and conform to the remainder of the ‘in’ group. Ms Brizendine’ theory can go some way to explaining women’s body anxiety: women can be vicious to each other. Women instinctively know each other’s psychological Achilles’ heel (because they might also suffer from the same body anxiety insecurity), and might use this to destroy her self-confidence. Fundamentally, what one woman says about other women’s physical form is a projection of how that woman feels about her own physical form. So a woman who is not happy in her own skin relieves herself of her own self-loathing by projecting her unhappiness onto the unsuspecting image of another woman. In the end, all this will do is further damage her own, already damaged, sense of self and possibly the self-esteem of the unsuspecting recipient. Remember, if a woman was happy in her own skin, it shows in what she says about herself, and other women. Ultimately, this will make her even more attractive, just as a negative or critical attitude can make a person seem less attractive. So, get rid of the idea that all women need to confirm to some specific body ideal; these attitudes are archaic relics from our past. Take that anxiety we all feel and convert it into an energy that can be used to uplift yourself and other women. Listen to that childhood voice that believes you are beautiful. Use new age survival instinct and positive mantras for you to quieten those negative thoughts. Ignore the unhelpful critiques of yourself and others. Resist the temptation of the Machiavellian delight and delicious anonymity that comes with being a cyber troll or social body image critic, and create a common conversation in your social circle of staying focused on pure positive body affirmation to reduce body anxiety in all of us.

It’s fun and a great way to make new friends while doing something that feeds your frontal cortex of your brain with the naturally occurring feel good hormones of dopamine and serotonin. Ask the people who you have contact with to help you raise your selfesteem. Treat each other with respect and be honest about all your feelings. P sychology tip: You might join a club or activity such as a team sport or service organisation to surround yourself with a supportive community.

H elp someone less fortunate in your community. Be a

volunteer or donate some of your time to someone to gain perspective about your own body image – it feels really good to help others. Whatever you do will be appreciated! P sychology tip: Volunteer at the burns unit at your local hospital, or maybe children with special needs (physical and cognitively impaired), this will give you greater insight and perspective about what body anxiety you might have about yourself. F orgive yourself for your body management mistakes.

Learn what you can from them and then let them go. Accept yourself the way you are, and then take the steps to improve those areas in your life you want to change. P sychology tip: Set aside mandated “Reflection Time” each day, where you have the sole purpose to worry as much or as little as you need. By being structured with your worry time you will become more effective with your problem solving skills. M ake a conscious choice to take responsibility for your

own self-esteem, and remember that while others might bear some influence on the way you feel about yourself, the true value you give yourself, is ultimately a choice which is up to you.

report was released last year from the Australian Longitudinal Study on Women’s Health (ALSWH) on our adherence to health guidelines. The data was collected from over 40,000 women randomly selected from the Medicare database and looks at a range of health indicators including diet. The major findings of the reports were: • The majority of women did not meet dietary guidelines for most food groups • The only exception was for intake of meat • The guideline to consume five serves of vegetables per day is the least likely to be followed • The consumption of extra foods is higher than recommended All core food groups except meat were under-consumed, however we are still getting plenty of kilojoules because our intake of foods classed as ‘extras’ is too high. These are the foods we can all enjoy from time to time, however the current average intake is more than four serves a day, which is just too much, and it’s taking the place of more nutritious foods. Eating enough fruits and vegetables gives people a better chance of getting the nutrients and fibre they need, and is linked to a reduced risk of cardiovascular disease, diabetes, obesity and certain cancers. The added bonus is researchers in Scotland have found a higher intake of fruit and vegetables really does give you a ‘healthy glow’. The pigments found naturally in fruits and vegetables can accumulate in your skin and change the pigmentation. The skin changes from eating more fruits and vegetables were rated by study participants as more attractive than a suntan. The most concerning statistic from ALSWH report was the low level of vegetables consumed, for example less than one percent of women aged 35-39 consumed five or more serves of vegetables a day. W

hen I first started WeightLoss Solutions Australia at the beginning of 2000, we had only the lap-band surgery to offer our patients. At that time, weight loss or bariatric surgery was still considered by many to be ‘taboo’, ‘a last resort’ or ‘dangerous’. Through developing a team-managed approach to caring for patients and consistent education programs, those original ideas have changed, and options such as the ones we offer are now seen as the life-changing solutions they are. One of the most exciting and rewarding elements of the past 14 years has been getting to know patients, and sharing their personal milestones and celebrating their successes; whether it be ‘I’m off my diabetes medication’ to ‘I don’t need my CPAP (a breathing monitoring device machine)’ to being able to play with the kids or shop in fashion boutiques where the only problem is the choice of designer labels – these types of successes are constant reminders to me that what our team does is changing people’s lives every day. I have so many very special patient memories that stand out for me and these are perpetual reinforcement that we need to strive for excellence daily and reach out to help all those who need to transform their lives for healthier, happier and more active futures every day. One patient in particular, who was always humble and discreet, and always a gentleman, was a man named Dom. He had a voice for radio and was determined to change his life in January 2001, just after turning 40. He set a date and a personal goal and never gave up. The GP he had visited had said to him “You need to lose weight. You know what to do!”. The attitude towards surgery among the medical community has changed now, but back then, in 2001, patients were still afraid to ask for a referral. Dom talked about obesity being ‘the last form of acceptable discrimination’. This statement would consistently shock me. It is 28 slim magazine summer 2014

never okay to discriminate against a person because of their size. Yet he would discuss how people changed their demeanour, regard and overall social acceptance of him as a ‘normal’ weight person and this had a profound impact on him and his evaluation of the ongoing discrimination in our community based on size alone. Dom recounted how his own mother (who was not aware that he had undergone surgery) could not understand how much weight he had lost. She lived in Sydney and he lived in Brisbane and she sent him a new pair of jeans for his birthday. He returned them to her to exchange them as they were too big, this happened a few times and each time he had to say “Mum, I’ve lost a bit more weight”. Dom ended up becoming a great advocate for the lap-band surgery and spoke at many seminars; sharing his success following surgery. His story was empowering as well as engaging and he would invariably have tears amongst the crowd as he recounted his story. Every seminar he would finish with “I wish you well on your weight loss journey and God’s speed”. I think of Dom every time we have new people walk cautiously through our doors to attend a seminar for the first time, recognising they need to do something to change their lives. He is a wonderful speaker; inspired many people, and is loved by many. I feel lucky to know him and to have been around to share his results. I could share patient stories with you forever; it is my absolute passion. hen P&G Prestige, the company that holds the license for Hugo Boss fragrances, were looking for a multifaceted woman to represent the new fragrance, Boss Jour Pour Femme they could not go past the woman who is known all over the world simply as Gwyneth. “Gwyneth embodies hard work, femininity and sophistication,” said Carla Luini, vice president of fashion brands for P&G Prestige. “She is a woman that has so many different roles, each and every day. From being an actor to being a mother, a writer, a businesswoman...” For her part, Paltrow said, “I have had a really good experience working for Boss, and I always say this, but it really is such a perfect job when you have kids.” “I can only really do one film a year now because of my family and how it all works out, so it’s so nice for me to be able to work for a great company and, you know, I’m going to Spain next week for two days so I still get to travel and meet really interesting people and have a work interaction. But it’s not like I am out of the house for three months,” added Paltrow. Paltrow’s parenting has often come under scrutiny in the media and online, with the star receiving criticism for everything from her children’s diet to her choice of transport. It is criticism that the actress says she pays little attention to. “The older I get, I realise it doesn’t matter what people who don’t know you think. It doesn’t matter. You’re wasting your energy,” she told the British magazine Red. “It’s like, if your partner comes to you – or your best friend – and says, ‘Listen, I want to talk about something you did that hurt me, or I think you could improve,’ sit down and listen to what they have to say. I love it; I have always loved it and have always had a wardrobe full of scent,” she said, admitting to having at least a hundred in her possession. “I think that Jour is the perfect name for it – it’s very uplifting,” she said of the new scent. And uplifting may be just what she needs given her regular morning routine. “I wake up at 7 a.m., I get [the kids] fed, and I get them dressed in their uniforms, any bits of homework are finished,” Paltrow explained. “I take them to school. She (Apple, now 9) gets dropped off at 8:25 a.m., and he (Moses, now 7) gets dropped off at 8:45 a.m.,

The Tan Commandments Follow these golden rules for a natural looking, beautiful glow. • Exfoliate prior to your tan, to help the formula apply evenly. • Have any hair removal done up to 48 hours prior to tanning. • Apply your tan to clean, dry, unmoisturised skin. • Wear gloves to prevent staining hands and nails. Healthy Bones If you want to look and feel fit, healthy and happy, better start with your bones.

A

s you get older, one thing you want to make sure of is that you have strong bones. Around 2 million Australians (about twice as many women as men) currently suffer from osteoporosis and this figure is expected to rise to 3 million by 2021, according to Osteoporosis Australia. That’s bad news because brittle bones means pain and disability and we all want to avoid that for as long as possible or, preferably, forever. So how do we do that? Some quick facts: the majority of bone mass is accumulated before the age of 20, with only minor accumulations in bone density until the age of 30. After 35, you lose up to one percent bone calcium every few years. Then, in the first few years after menopause, most 44 slim magazine summer 2014

women experience rapid bone loss of 10 to 15% within five to 10 years. Both men and women continue to lose 0.5 to 1% of bone throughout life. So building bone early is crucial which is why it’s great to take part in school sport and get plenty of healthy foods including vegetables, bony fish and low fat dairy. But, throughout your life, caring for your bones with good food and exercise is also important. Sources of bone-building calcium include calcium-supplemented soy drinks, salmon, sardines, low fat milk, cheese and yoghurt, while there’s evidence that eating plenty of green veggies and less meat helps keep the blood alkaline and that’s good for bones.

Calcium requirements at different stages of life The amount of calcium you need depends on your age and sex. The highest daily requirements are for teenagers (a period of rapid bone growth) and for women over 50 and men over 70. We achieve our Peak Bone Mass – the point at which our bones are at their highest density – by our early twenties. Nearly 40% of our Peak Bone Mass is acquired during puberty. Achieving a high Peak Bone Mass during these younger years can help maintain better bone health throughout life, and an adequate calcium intake is therefore essential for children and teenagers. In adulthood, adequate dietary calcium is vital to maintain bone strength. For women in particular, menopause is a time of more rapid bone loss – calcium requirements increase at this stage of life. In older adults, calcium is absorbed less effectively from the intestine and more can be lost through the kidneys, so calcium intake needs to be maintained at a higher level.

Avoid excessive intake of salt and alcohol, which may increase calcium excretion, so your bones won’t be getting what they need even if you think you are eating enough calcium. Substances that may reduce the amount of calcium which is actually absorbed, when taken in large quantities, include tannins in tea, oxalate in spinach; caffeine in coffee, tea and cola drinks; and phosphate without calcium in soft drinks, according to Osteoporosis Australia. Current research suggests regular weight-bearing exercise including walking contribute to bone health. Yoga, weight-training and other resistance-type exercises may also be useful. Exercise physiologist Mark McKeon, author of Life Tips, says you can use your own weight for resistance exercises that strengthen bones. In other words, do squats, press-ups, burpees, chin-ups and anything else you can think of where you are working against your weight three times a week for at least 15 minutes. Regular walking is also beneficial, as is jumping rope. Having stronger muscles can also reduce the chance of falls and fall-related fractures. According to Sydney’s Garvan Institute, 75% of how good your bones are at 20 is genetically determined. So, besides knowing what drugs you are taking, knowing your parents’ medical history can help your GP in helping you protect your bones. Everyone over 40, or with a poor diet, should also consider taking a daily calcium supplement while anyone who doesn’t get much sunshine should have their Vitamin D level tested. It’s needed for calcium absorption.

This can occur after prolonged bed rest or in people who are wheelchair bound and can even impact on astronauts due to the weightlessness of space.

It may seem like ‘gluten free’ is just the latest health fad, but coeliac disease is a serious condition, and many Australians don’t even know they have it. Here is everything you need to know.

What is coeliac disease? If you suspect you may have coeliac disease The symptoms of coeliac disease vary considerably. Some people experience severe symptoms while others are asymptomatic (they have no obvious symptoms at all). Symptoms can include one or more of the following:

• Gastrointestinal symptoms e.g. diarrhoea, constipation, nausea, vomiting, flatulence, cramping, bloating, abdominal pain, steatorrhea • Fatigue, weakness and lethargy • Iron deficiency anaemia and/or other vitamin and mineral deficiencies • Failure to thrive or delayed puberty in children • Weight loss (although some people may gain weight) • Bone and joint pains • Recurrent mouth ulcers and/or swelling of mouth or tongue • Altered mental alertness and irritability • Skin rashes such as dermatitis herpetiformis • Easy bruising of the skin -----People who experience any of the following should also be screened for coeliac disease:

• Early onset osteoporosis • Unexplained infertility • Family history of coeliac disease • Liver disease • Autoimmune disease e.g. Type-1 diabetes, autoimmune thyroid condition Importantly, treatment with a strict gluten free diet leads to small bowel healing, resolution of symptoms and a reduction in the long-term risk of these complications. Although symptoms can vary considerably in coeliac disease, everybody with the condition is at risk of complications if they do not adhere strictly to treatment with a gluten free diet. There is no correlation between symptoms and bowel damage so even if you are asymptomatic (you have no obvious symptoms), damage to the small bowel can still occur if gluten is ingested. This means everybody with coeliac disease, irrespective of the severity of their symptoms, needs to adhere strictly to a gluten free diet.

In people with coeliac disease, the immune system reacts abnormally to gluten (a protein found in wheat, rye, barley and oats), causing small bowel damage. The tiny, finger-like projections which line the bowel (villi) become inflamed and flattened. This is referred to as villous atrophy. Villous atrophy reduces the surface area of the bowel available for nutrient absorption, which can lead to various gastrointestinal and malabsorptive symptoms. Symptoms can also be caused by inflammation in other parts of the body. A number of serious health consequences can result if the condition is not diagnosed and treated properly.

Who gets coeliac disease?

Coeliac disease affects people of all ages, both male and female. You must be born with the genetic predisposition to develop coeliac disease. The most important genes associated with susceptibility to coeliac disease are HLA DQ2 and HLA DQ8. Either one or both of these genes are present in virtually every person with coeliac disease. While 30 percent of the population carry one or both of these genes, only 1 in 30 of these people (approximately) will get coeliac disease. A first degree relative (parent, sibling, child) of someone with coeliac disease has about a 10% chance of also having the disease. If one identical twin has coeliac disease there is an approximate 70% chance that the other twin will also have coeliac disease (but may not necessarily be diagnosed at the same time). Environmental factors play an important role in triggering coeliac disease in infancy, childhood or later in life.

How common is the condition?

Coeliac disease affects at least 1 in 100 Australians. However approximately 75% currently remain undiagnosed. This means that approximately 160,000 Australians have coeliac disease but don’t yet know it. More and more people are being diagnosed with coeliac disease. This is due to both better diagnosis rates and a true increase in the incidence of coeliac disease.

Can coeliac disease be cured?

People with coeliac disease remain sensitive to gluten throughout their life, so in this sense they are never cured. However, a strict gluten free diet does allow the condition to be managed effectively. A strict, lifelong gluten free diet is currently the only recognised medical treatment for coeliac disease. By removing the cause of the disease, a gluten free diet allows the small bowel lining to heal and symptoms to resolve. As long as the gluten free diet is strictly adhered to, problems arising from coeliac disease should not return. Relapse occurs if gluten is reintroduced into the diet.

What are the long term risks of undiagnosed and untreated coeliac disease?

The long term consequences of untreated coeliac disease are related to chronic systemic inflammation, poor nutrition and malabsorption of nutrients. Fortunately, timely diagnosis of coeliac disease and treatment with a gluten free diet can prevent or reverse many of the associated health conditions.

Why a medical diagnosis is important

As coeliac disease is a serious medical condition with lifelong implications, a definitive diagnosis is essential. The gluten free diet is not a trivial undertaking and involves lifestyle changes and learning new skills such as reading and interpreting food labels. It should only be undertaken after the diagnosis of coeliac disease has been properly medically established. If a gluten free diet has already been adopted prior to diagnosis, the tests used to diagnose coeliac disease are unreliable, and can be falsely negative. By obtaining a proper diagnosis, you can be assured that your symptoms are caused by coeliac disease (and should therefore improve once the gluten free diet is established) and not by another more sinister condition.

Additional benefits of diagnosis

•A gluten free diet has positive implications for health by reducing the long-term risks associated with coeliac disease. People who have been properly medically diagnosed are more likely to maintain the strictness required to remain healthy. • I n the future, it is likely that any potential non-dietary therapy for coeliac disease, such as a therapeutic vaccine, will only be available to those who have coeliac disease properly proven by biopsy. •O n diagnosis of coeliac disease, screening for complications and associated conditions such as osteoporosis or autoimmune diseases should occur. Being diagnosed properly will ensure this important medical assessment takes place. • I mmediate relatives should be screened for coeliac disease once a family member is diagnosed. A proper diagnosis will ensure this occurs.


WHAT IF IT’S NOT COElIAC DISEASE?

Once coeliac disease has been thoroughly investigated and ruled out, your doctor can further explore other causes of symptoms. There are a variety of potential explanations. Depending on the circumstances, your doctor may feel it important to perform some additional tests. BEWARE OF UNORTHODOX DIAGNOSIS TECHNIQUES

There are a number of tests and treatments for allergy, intolerance and coeliac disease that are used in the absence of any scientific rationale. These tests and treatments have been shown to be unreliable when subjected to careful study. Unproven testing methods provide misleading results, delay correct diagnosis and lead to unnecessary and ineffective treatment. The Australasian Society of Clinical Immunology and Allergy (ASCIA) advise against the use of such tests for diagnosis or to guide medical treatment. Such methods may include stoolbased tests, Vega testing, iridology, hair analysis or the inappropriate use of tests for food-specific immunoglobulin. HOW DO I GET DIAGNOSED?

The tests for coeliac disease are simple – just follow the steps below... 1. Keep eating gluten Do not commence a gluten free diet prior to being tested for coeliac disease. If a gluten free diet has already been adopted, the tests used to diagnose coeliac disease are unreliable, and can be falsely negative. If gluten has been removed from the diet, a normal diet must be resumed for at least six weeks prior to testing. During this ‘gluten challenge’, a minimum of four slices of wheat based bread (or equivalent) should be consumed each day (for adults) (two slices of wheat based bread each day for children). It is important the gluten challenge is carried out properly to ensure reliable test results. 2. Blood tests are used for screening Blood tests (coeliac serology) are used to screen for coeliac disease. Coeliac serology measures antibody levels in the blood which are typically elevated in people with untreated coeliac disease, due to the body’s reaction to gluten. These tests can be less reliable in children under the age of four years, as their antibody levels can fluctuate. It is suggested the antibody tests be performed on two occasions three months apart. Importantly, a diagnosis of coeliac disease SHOULD NOT be made on the basis of a blood test alone. A positive blood test always needs to be followed by a small bowel biopsy to confirm the diagnosis. 3. A small bowel biopsy is essential to confirm diagnosis A diagnosis of coeliac disease can only be made by demonstrating the typical small bowel changes of coeliac disease (villous atrophy). This involves a gastroscopy procedure in which several tiny samples (biopsies) of the small bowel are taken. A gastroscopy is a simple day procedure done under light anaesthetic sedation that takes about 10 minutes. In the majority of cases, the bowel damage present in those with untreated coeliac disease is not visible to the naked eye. The biopsies are examined under a microscope to confirm the presence of villous atrophy.

eople unaware of their sensitivity towards gluten, may not be enjoying life to the fullest as they are dragged down by the negative implications this allergen has on both health and their lifestyle. Gluten intolerant individuals generally experience bloatedness, bowel discomfort and occasionally… feel gaseous. In extreme cases, this can cause chronic inflammation of the intestine, an autoimmune reaction experienced by those suffering from coeliac disease (CD). These symptoms are the body’s natural way of telling you it is unwell as it cannot break down gluten generally found in wheat and other grains. The food industry has been catering their products for health conscious individuals – coeliac disease sufferers or those on a gluten-free diet. Take a walk through any supermarket, health food store or dining bistro these days and you’re bound to discover a selected assortment of gluten-free, wheat-free products, ranging from cakes, breads and pastries on the menu. Whilst you might love the wide variety of gluten-free products available on the market, you may not be completely pleased at its possible ‘vitamin-free’ or ‘nutrient-free’ composition hidden in these foods. Whole grains are an essential source of proper nourishment including essential nutrients such as fibre, Vitamin E, B group vitamins, proteins and a whole lot more. Unfortunately, the removal

Core fitness If you want to stay fit and active – and avoid injury – you need to work on your “core”.

T

hese are your mid-section muscles and fitness experts say they are essential for good posture and preventing pain. When we lift a box, pick up a child, or even sit up straight, the muscles we are using are our middle section or ‘core’. Core muscles include all the muscles of the trunk – front and back, inner and outer, that help with stabilising and moving the spine and pelvis. Our core muscles help support the spine and the pelvic floor, which makes them crucial muscles for women, says personal trainer Stephanie Stone, of Stepping Stone Health and Fitness (steppingstone.net.au). “They are also responsible for posture, balance and stability,” she adds. Train your core and you’ll have less risk of a bulging disc, or a hernia, or simply of pains and strains. You’ll also develop a flatter, more toned looking stomach although, of course, the aesthetic benefits aren’t the only reason to work your core!

Going to an exercise class, at least until you learn the proper technique, is always a good idea.

So how do you know if you have poor core muscles?

Unless you train them, or are especially active and remember to contract your abdominals before you lift or twist, they probably won’t be that strong. Your core muscles may also need work if you regularly find that you get pain in your lower back or even your knees. The reason is that these muscles are carrying the load for your lazy core muscles. While for many years the fitness industry recommended abdominal curls for stronger abdominal muscles and lower back support, now there’s a whole range of exercises you can do for your core. You can even get a fitness ball and use that to reduce your stability while you do exercises, thereby forcing your core to work. Start slowly though.

Stretch for strength

Two of the most popular exercise trends of the last 10 years have been Pilates and yoga, both of which work overall core strength. With Pilates, you perform very precise exercises that strengthen the body from the inside out. These can be on a mat, or with exercise equipment in a studio. Like yoga, Pilates is not just an exercise regime but also a mind-body program that stresses proper breathing while you do the exercises. Nearly all yoga poses, from standing to twisting, require abdominal strength. This is the reason that all types of yoga develop core muscles. Going to an exercise class, at least until you learn the proper technique, is always a good idea for both of these disciplines but you may also like to get a DVD you can use at home in between classes or when you feel you are proficient. Alternatively, there are some very simple core strengthening exercises that can be performed by anybody, at home, in the gym, or even in a shopping queue. For best results, you need to practice them two to three times a week, using an exercise mat for the floor poses. If you can’t get to the maximum number of repetitions straight away, don’t worry. Just slowly increase the amount you can do until you reach the target. Once your core muscles are feeling stronger, you should notice lots of benefits. You’ll stand taller, you’ll feel more comfortable, you’ll have a flatter stomach, and you’ll be less at risk of sustaining an injury that will put you out of action for weeks.

f you ever wondered how your thin friend eats so much food and stays slim while you only have to look at a piece of chocolate cake to gain a gram, we have one word for you – metabolism. The good news is that you can change the rate at which you burn energy, says exercise physiologist and motivational coach Mark McKeon (mckeon.com.au). “Genetics are the most important factor in Basal Metabolic Rate (BMR, the amount of energy we expend while at rest) and there’s nothing we can do about that. But building muscle, staying active and increasing the quality of food you eat increases your BMR.” Perhaps the number one strategy for raising your metabolism is increasing your amount of muscle mass. “Muscle burns kilojoules at rest, fat does not. In fact muscle is eight times more metabolically demanding than fat cells. This is why lean but well-muscled people seem to be able to eat anything without putting on weight,” says McKeon. Any increase in activity and muscle mass will result in an improvement in kilojoule burning although exercises that build muscle – squats, press-ups, burpees and, of course, weight training with dumbbells or at the gym – will really pay results. How much weight/resistance exercise, how often, will depend on how active you have previously been but be aware that muscle weighs more than fat so don’t look at the scales, look at your clothes to gage results. The quality of food you eat is important to helping your metabolism run efficiently. For example, dietary fat is very easy to process and has very little thermic effect, while protein is hard to process and has a much larger thermic effect. Nutritionist Zoe Bingley-Pullin, author of Eat, Taste, Nourish, says your body has to work harder to break down the fibre and complex carbohydrates in whole grains and vegetables than it does to break down fibre-free refined flours or simple carbs such as honey and sugar. “Another big factor is water. Your body needs two to two and a half litres of it for metabolic processes,” she says. Since metabolism is governed by thousands of separate chemical reactions, making sure your body has a constant supply of nutrients may also be crucial. Without vitamins such as B2, B3, B5, B6 and Vitamin C metabolism can become inefficient and sluggish.

Other ways to boost your metabolism

Relax. Stress can interfere with the efficiency of your metabolism. Try meditation. Raise your body temperature. BMR increases by about seven percent for a 0.5 degree Celsius increase in body temperature. This may be why it’s easier to stay slim in summer. It could be because we’re more active and eat lighter foods too. “Conversely, cold external temperatures, increase BMR as we try to keep our central system warm,” says McKeon. “When we shiver, we definitely boost our metabolic rate as every mitochondria in every cell in your body generates greater energy production.” Exercise in the morning and lift your metabolic rate by about 10% for another four hours.

The cold side of cold cuts

A bite of your BBQ meat lover pizza might seem like your tastebuds are entering Nirvana after a weekend of debauchery and cocktails, but ham, salami, roast beef and other deli meats are often poor quality, packed with sodium and made from animals raised on hormones and antibiotics. The World Cancer Research Fund recently looked at 7,000 clinical studies covering links between diet and cancer, and strong evidence is emerging that eating processed meat is linked to an increased risk of bowel cancer and a suggested link with lung, stomach, prostate and oesophageal cancer. So put your salami sanga down and opt for fresh meat like organic chicken, fish or beef.

Nothing sweet about you

Yes I get it: you’re sweet enough so you cut the sugar and go artificial. Rookie mistake. Not only have some fake sweeteners been linked to people not losing weight but they can be potentially dangerous to your health. Natural News recently reported aspartame (which is found in most diet drinks) “has been linked to a number of diseases, can impair the immune system, and is even known to cause cancer.” In one study where rats were given aspartame, up to 67% of female rats developed tumours the size of golf balls. Here’s a simple rule: if it is made in a lab, it’s probably better to avoid it, which is why a halfteaspoon of sugar in your coffee, a dash of honey, or stevia are better options than a packet of the fake stuff. Margarine, spread it – away

Ever flown to the land of croissant and seen those taut slim French girls spreading butter on their pastries? It’s not rocket spread science: most margarines are simply not healthy, due to the process of hydrogenation, which creates trans fats, which causes inflammation by damaging the cells lining your blood vessels, increasing your risk of cardiovascular disease, cancer, degenerative diseases and putting fat on your thighs. Our body needs natural good fat in our diet to function and lose fat, so go for organic butter, olive oil spread, avocado, coconut or macadamia oil as the healthier replacement.

MSG – the masked bandit

When you get the urge to drag your salivating mouth to China Town for a bowl of beef chilli dumplings, be warned. The allure of many of these neon lit eateries is that they often promise food that is delivered fast, cheap, salty and tasty… and then the symptoms strike. Symptoms of MSG intolerance can include dizziness, headaches and heart issues. MSG is not just found in Chinese takeaway, as many people believe, it is actually found in many pantry items. Think corn chips, bottled sauces, instant noodles, commonly masked under the “flavour enhancer” ribbon. Always read labels for this hidden bomb of flavour as it goes under many names or just avoid savoury packet foods altogether.

The white stuff

It’s unfortunate that so many people think the right fuel for starting their day means grabbing a doughy white roll or dunking their spoon in a bowl of sugary cereal. When you eat a lot of sugar, the carbohydrates get used up quickly and your blood sugar shoots up. As a result, your brain quits making orexin, and without this neuropeptide you will feel like crawling under your desk to have a nap. Turn the food pyramid around again and avoid white breads, cereals or breakfast bars and go for a better choice with protein, like scrambled eggs with a fetta avocado mash. TOP WEIGHT LOSS TIPS W hen it comes to exercise, just do it. No excuses. Exercise is

complimentary to weight loss. Remember the 80:20 rule: 80% of weight loss is what you eat whilst 20% is the toning and tightening due to exercise. K eep a food diary. Either with pen and paper or an app on your Smart Phone. The food diary will make you aware of what you are actually eating and drinking, when you are eating and what changes are necessary so that you get the results you need. D on’t count calories. You need nutrient rich, portion controlled foods most of the time that meet all your dietary requirements. So you had a break out? Get over it. Pick yourself up, dust yourself off and move on. Get back on track at the very next meal or snack. Do not try to compensate for your break out by “fiddling” your next day’s food intake. Remember you will still lose weight with one indulgence or non-compliance per week; two indulgences per week and your weight will stay the same; however three indulgences per week and you will gain weight.

Food, glorious food I classify food three ways – ‘everyday foods’; ‘sometimes foods’ and ‘special occasion foods’. ‘Everyday foods’ are foods that you enjoy everyday; foods that make up the bulk of your diet and are staples in the shopping cart. ‘Sometimes foods’ are a special treat or indulgence that you might have once per week. ‘Special occasion foods’ are exactly what they sound like. Foods that appear on your plate in controlled quantities on special occasions. The trick here is to decide if the occasion is “special” enough to allow you to indulge and accept the penalty of either not losing weight or even gaining weight.

What is breast reduction for men?

Also known as gynaecomastia, breast reduction for men is the surgical correction of over-developed or enlarged breasts in men. The condition of over-developed or enlarged breasts in men is common in men of any age. It can be the result of hormonal changes, heriditary conditions, disease or the use of certain drugs. Gynaecomastia can cause emotional discomfort and impair your self-confidence. Some men may even avoid certain physical activities and intimacy simply to hide their condition. Gynaecomastia is characterised by: • Excess localised fat • Excess glandular tissue development • A combination of both excess fat and glandular tissue • Gynaecomastia may be present unilaterally (one breast) or bilaterally (both breasts)

Male breast reduction candidates

Gynaecomastia surgery is a good option for you if: • You are physically healthy and of relatively normal weight • You have realistic expectations • Your breast development has stabilised • You are bothered by the feeling that your breasts are too large Adolescents may benefit from surgery, although secondary procedures may be needed in the future should breast development continue. Male breast reduction surgery is best performed on: • Men whose condition cannot be corrected through alternative medical treatments • Healthy individuals who do not have a life-threatening illness or medical conditions that can impair healing • Non-smokers and non-drug users • Men with a positive outlook and specific goals in mind for improving the physical symptoms of gynaecomastia Breast reduction for men is a highly individualised procedure and you should do it for yourself, not to fulfill someone else’s desires or to try to fit any sort of ideal image.

What to expect during your consultation

The success and safety of your gynaecomastia procedure depends very much on your complete candidness during your consultation. You’ll be asked a number of questions about your health, desires and lifestyle. 60 slim magazine summer 2014

What happens during gynaecomastia correction surgery?

Plastic surgery to correct gynaecomastia is technically called reduction mammaplasty, and reduces breast size, flattening and enhancing the chest contours. In severe cases of gynaecomastia, the weight of excess breast tissue may cause the breasts to sag and stretch the areola (the dark skin surrounding the nipple). In these cases the position and size of the areola can be surgically improved and excess skin may be reduced. Anaesthesia: Medications are administered for your comfort during the surgical procedure. The choices include intravenous sedation and general anesthesia. Your doctor will recommend the best choice for you. Liposculpture: In cases where gynaecomastia is primarily the result of excess fatty tissue, liposuction techniques alone may be used. This requires insertion of a cannula, a thin hollow tube, through several small incisions. The cannula is moved back and forth in a controlled motion to loosen the excess fat, which is then removed from the body by vacuum suction. There are various liposculpture techniques that may be used; including laser lipolysis – the technique most appropriate in your case will be defined prior to your procedure. Excision: Excision techniques are recommended where glandular breast tissue or excess skin must be removed to correct gynaecomastia. Excision also is necessary if the areola will be reduced, or the nipple repositioned to a more natural male contour. Incision patterns vary depending on specific conditions and surgical preference. Sometimes gynaecomastia is treated with both liposculpture and excision. Will there be scars? Any surgical treatment to correct gynaecomastia will require incisions. While most incision lines are concealed within natural contours, some may be visible and are a necessary result of breast reduction surgery. Bikram yoga, developed by Bikram Choudhury, is a form of hatha yoga which takes place in a room heated to 40 degrees Celsius with around 40 percent humidity. The 90 minute class consists of a series of 26 postures, which are repeated twice and held for between ten seconds to a minute. The heated room helps you sweat out the chemical imbalances produced in daily living and return the body to a purer state. The hot room also helps relax the muscles as they are stretched, clear the circulatory system by thinning the blood, and improve the cardiovascular workout by increasing the heart rate even though the breathing remains regular. Tina Lynton, a devotee of Bikram, says at first she was surprised by the nature of the practice. “A friend told me about it but I didn’t really take much notice, as I had tried other types of yoga and hadn’t really enjoyed them. I like to sweat when I exercise,” Tina says. “Then another friend mentioned it to me and I thought I had better go and see what it was all about. I have loved it ever since. I have always been an active person and loved any kind of sport or physical activity or challenge. However, Bikram is different to anything I had done before as it is not just a physical workout – it is a workout for your mind as well.

What do you specialise in? I am a Diploma Qualified Beauty Therapist and Electrologist, and am currently studying to be a Dermal Therapist. I am the Electrologist at e-sab. (These can also be used to apply the health-giving principles of the Mediterranean diet to other kinds of cuisine, she says). The commandments are: One Use olive oil as the main added fat (aim for around 60 mls/day).

Seven Eat yoghurt everyday (about 200g) and cheese in moderation (about 30 to 40 grams per day).

T

he Mediterranean Diet is one of the most respected and prescribed diets in the world. It has been rigorously tested for more than 60 years, and supporters – of which there are many – say the results are indisputable. In fact it is claimed that the diet is as effective as drugs in treating heart and diabetes (and can prevent both conditions) aids weight loss, promotes longevity and slows the progress of Alzheimer’s. The book’s author Dr Catherine Itsiopoulos has spent her working life researching the diet. In this, her first book for a general audience, she offers an accessible and scientifically proven version of the diet specifically for Australians. Drawing on the food traditions of her Greek heritage, Dr Itsiopoulos gives us 80 beautifully photographed recipes, eating plans and nutritional advice, as well as sharing the evidence as to why this remains the world’s most effective diet for long-term health outcomes and weight management. The key components of the diet are: olive oil, leafy greens, eggs, fruit and nuts, legumes, fermented dairy products (yoghurt and fetta), seafood, a small amount of red meat and a miniscule amount of sugar. “The book was inspired by my cultural heritage and fuelled by my desire for scientific knowledge,” she writes. “It’s the culmination of the lessons I learned as a child growing up in a Greek household and the knowledge I have gained from the professors I’ve met and the scientific papers I’ve read.” Catherine says the Mediterranean Diet consists of a high intake of vegetables (particularly leafy greens and tomatoes) and fresh fruit; wholegrains (mostly sourdough bread rather than pasta), olive oil as the main fat, cheese in moderation (goat’s cheese), yoghurt, nuts, fish and seafood, small portions of meat and moderate amounts of wine drunk only with meals. “The diet satisfies hunger and our cravings for variety. It celebrates food and the pleasures of eating.” 70 slim magazine summer 2014

Six Eat smaller portions of meat, less often (beef, lamb, pork and chicken) no more than once or twice a week.

Two Eat vegetables with every meal (include 100g leafy greens and 100g tomatoes, and 200g other vegetables/day). Three Include at least two legumes meals (250g serve) per week.

Four Eat at least two servings of fish (150-200g serves) per week and include oily fish: for example Atlantic and Australian salmon, blue-eye trevalla, blue mackerel, gemfish, canned sardines, and canned salmon. Canned tuna is not as high in the important fish oil Omega-3, but still a good choice to include in your fish serves. Five Eat fresh fruit every day and dried fruit and nuts as snacks or dessert.

Eight Include wholegrain breads and cereals with meals (aim for 3-4 slices of bread per day). Nine Consume wine in moderation (one standard drink a day, which is about 100mls), always with meals and don’t get drunk. Try and have a couple of alcohol free days a week.

Ten Have sweets or sweet drinks for special occasions only.


Rocket, Pear, Parmesan and Pine Nut Salad (Salata me oka) serves 4

100 g rocket leaves, washed Âź cup (40g) pine nuts and slivered almonds, lightly toasted in a dry frying pan 1 brown or Corella pear, thinly sliced 2 tablespoons olive oil 2 tablespoons balsamic vinegar (or red or white-wine vinegar) freshly milled sea salt, to taste white pepper, to taste Âź cup (25g) shaved parmesan As a meal for one, use half the quantities given here and add a small can (95g) of tuna or salmon. Toss rocket, pine nuts and almonds and pear slices until well combined. Toss through olive oil and vinegar, and season to taste. Just before serving, toss through the shaved parmesan.

recipes Caramelised Orange Peel Spoon Sweet (Portokali Gluko) 3 oranges or ruby grapefruit 24 whole cloves 2¼ cups (560 g) sugar 1¼ cups (310 ml) water 1 tablespoon fresh lemon juice 1 metre cooking twine, to thread orange rind Greeks make preserves of everything, from sour and salty pickles to super-sweet caramelised fruits. This spoon sweet is a favourite of mine and I often enjoy a small serve with a Greek coffee in the afternoon or evening when I need a sweet hit without the fat. Serve them on a small plate with some of the syrup, a cold glass of water and a Greek coffee! Wash oranges and lightly grate the outer skin to remove the waxy surface layer. Cut each orange into 8 pieces. Remove the majority of the pulp from each piece, retaining the peel. (You can snack on the pulp while you work.) Place the orange peel in a medium saucepan, cover with cold water, place over high heat and bring to boil, then reduce heat and simmer for 5 minutes. Drain and repeat the previous step two more times. Drain, cool slightly, then use a knife to make a small hole in each piece of peel. Thread 6 peel pieces each onto 4 lengths of twine, then tie the ends together to secure the peel. Push a whole clove into each piece of peel. Combine sugar and water in a larger saucepan and stir over medium heat until sugar dissolves. Add lemon juice and stir. Carefully lower the 4 chains of peel into the saucepan and simmer for 20-25 minutes or until the syrup thickens enough to coat the back of a wooden spoon. Remove from heat and cool well. Place peel and syrup in a sterilised jar with an airtight lid and store in the refrigerator until required.

Green Runner Bean Casserole (Fassoulakia me Karota kai Patates Yiaxni)

¼ cup (60 g) olive oil I medium onion, finely chopped 1 large carrot, cut into thin strips 500 g green runner beans, ends removed and cut in 2-3 pieces 1 large potato, cut into thick slices 250 ml puréed tomato 2-3 sprigs fresh flat-leaf parsley, finely chopped 2 cloves garlic, crushed freshly milled sea salt, to taste freshly ground black pepper, to taste dried oregano, to taste

This dish is best prepared when green runner beans are in season and are fresh and crisp. Heat olive oil in a large, heavy-based saucepan over medium heat and sauté onion for 1-2 minutes. Add carrots and sauté for a few seconds. Add beans and sauté for 5 minutes. Add potato and sauté for a few seconds. Add tomato, parsley, garlic, season to taste and stir well. Add enough hot water to cover the vegetables and simmer for 1 hour or until vegetables are cooked. Taste, add oregano to taste, adjust seasoning and serve. Hosted by Olivia Scott, the lunch was attended by Gold Coast’s best and brightest, dressed to the nines and ready to party. Guests dined on a three course lunch with divine wines and champagnes. When Toyota dips into its historic paint palette for the 2014 FJ Cruiser, SLIM publisher Kate Bennett falls in love all over again.

didn’t think it was possible to love Toyota’s FJ Cruiser any more, but they’ve done it with the new colour palette for 2014. Among the six new colours is Retro Blue, based on the original blue paint used for the vehicle that inspired the FJ Cruiser – Toyota’s famed LandCruiser FJ40 which was sold in Australia during the 1960s, 70s and early 80s. Toyota Australia’s executive director sales and marketing Tony Cramb told us FJ Cruiser’s success is due to its off-road abilities, suitability for everyday driving and flexibility for all types of activities. “Motoring enthusiasts love the FJ Cruiser because it inherits the DNA and pays homage to several of the most compelling stylistic themes of the original LandCruiser FJ40,” Mr Cramb said. “They also appreciate the FJ Cruiser being simple and rugged with the ability to traverse tough terrain and deliver full-scale off-road driving performance.” Toyota engineered the FJ Cruiser for serious off-road driving capability with Australian engineers playing a leading role in developing the ride and handling to meet the demands of local drivers and conditions. The calibration of the vehicle’s shock absorbers and power steering were retuned to meet requirements for a sporty ride and sharp handling on bitumen while retaining a soft ride in off-road conditions. Improvements to reduce noise, vibration and harshness, now adopted globally, were prompted by Australia’s coarse-chip road surfaces. The FJ Cruiser is also functional with side access doors that open 90 degrees in clamshell fashion for easy access to the rear seat. At the rear, a swing-up glass hatch opens independently of the side-hinged tailgate, making it easier to store or retrieve items from the luggage area. Now do you see why I love this car?

FJ FACTS F J Cruiser has a powerful and responsive DOHC 4.0-litre V6 petrol engine that incorporates dual independent variable valve timing with intelligence (variable phasing for both the intake and exhaust cams). T he 200kW, 380Nm engine is driven through a five-speed automatic transmission. FJ Cruiser features part-time 4x4, an electrically activated rear differential lock and switchable active traction control technology. I t features impressive clearance – a 31-degree departure angle and 29-degree break-over angle are the best for any vehicle in Toyota’s local 4WD range. The FJ ground clearance is 224mm. T he FJ Cruiser’s comprehensive standard safety package provides six airbags, including front seat-mounted side airbags and roll-sensing side curtain airbags for both rows of seats.
