MTM Health Team Product Terms and Conditions 1. I authorize MTM to immediately withdraw monthly fees for the product(s) I have selected, using the payment method I have identified. 2. To change my method of payment or the authorized products, I understand that I must submit a new Order Form to MTM. In the new form's note section I shall denote the words “PAYMENT INFORMATION CHANGE� and the ORIGINAL ORDER NUMBER. If more than one such agreement has been submitted; I agree that the most recent agreement will supersede all previous agreements. MTM reserves the right to change its prices associated with its products and services in its sole and absolute discretion. By physically or electronically signing the Product Order Form, a Client agrees to abide by all amendments or modifications that MTM elects to make. Amendments shall be effective upon notice to all Clients that the Agreement has been modified. 3. This Agreement will remain in effect until you: (i) elect to alter or change any aspect of this Agreement by submitting a new signed product purchase agreement; (ii) send, in writing, or by email within your Health Coach Account; your cancellation of your participation or Product Order Form to MTM at 340 South Lemon Avenue. # 7012, Los Angeles, California 91789 USA (Notice must include your signature, printed name, address, and Member Account Number), or (iii) stop payment of any withdrawals by MTM by notifying your issuing bank at least five (5) business days prior to the scheduled charging of your account. If a cancellation notice is received fewer than five (5) business days prior to the scheduled charging date, cancellation will become effective in month following the month in which your notice of cancellation was received by MTM. Alaska Residents: You may cancel this Agreement without penalty or obligation within five calendar days of the date of the Agreement and receive a full refund. Kentucky Residents: You may cancel this Agreement without penalty or obligation within thirty calendar days of the date of the Agreement and receive a full refund. If you cancel after thirty business days, you will be entitled to receive a pro rata refund without deduction for benefits received. New Hampshire Residents: If you cancel this Agreement more than three business days after its execution, you are entitled to a pro rata refund of your Membership fees without deduction for benefits received. Wisconsin Residents: You may cancel this Agreement within three months or within three business days of your first purchase of your Global Health Care Membership, whichever occurs first. If you cancel within the first fifteen calendar days of your Membership, you will receive a full refund of your Membership fee. If you cancel within three months of

this Agreement or within three business days of your first purchase (whichever occurs first), you will be entitled to a prorated refund of your Membership fee, without deduction for benefits received. 4. Global Health Care Members may purchase the goods and services described at MTM’s web sites or the web sites of MTM Health Coaches and Health Team Publishers. Such goods and services may only be purchased through such web sites without opportunity for inspection. You may be required to pay freight costs, handling fees, and other costs incidental to the purchase of such goods or services. The goods and services available through a Global Health Care Membership are purchased from third party vendors, partners and providers and not MTM. MTM makes no representation to the quality of such goods or services provided by a third party vendor, partner or provider. We agree to provide you with all the information that has been made available to us about each product or service. 5. I agree that it is my sole responsibility to choose which product or service I purchase and that by not electing to participate in all or any of these products or services offered does not constitute a breach of this agreement. I further agree that I will not be eligible for a full or partial refund of my Global Health Care Membership Fee and Monthly Fees as a result of my non-participation in any of the products or services offered by MTM. 6. I agree that due to the inherently fluctuating costs in the healthcare industry, the published price for each Global Health Care Treatment Package, Product or Service is not guaranteed to all Global Health Care Members. I further agree that MTM will make every available opportunity to offer me the best possible price but that prices may increase above the initially published price due to timing, limited inventory availability, current health, and any other industry fluctuations. 7. The MTM Health Team Business System (HTBS). If you have purchased any HTBS Products, Solution Packages or Services, instructions regarding set up, access or use of your HTBS will be delivered to you at the email address you listed on the HTBS agreement. Your use of the HTBS Products, Packages or Services is subject to additional terms, conditions and policies of use which you will be able to review prior to accessing. Such terms, conditions and policies of use are incorporated into and made a part of this agreement, by this reading and understanding of the entire agreement.
