LMH Community Care Navigators Reduce Readmission Rates Since LMH started its community care navigator program, the hospital has lowered 30-day readmission rates among the uninsured, Medicaid and chronic disease population it serves – in some cases by as much as 68 percent. By creating low cost, high impact connections between hospital care, primary care, specialty care, home care and community resources, LMH’s community care navigators are coordinating patient care for better outcomes, lower costs and higher quality health care. The most recent 12-month data shows a 9 percent hospital-wide drop in 30-day readmission rates for acute myocardial infarction (heart attack) and 7 percent for heart failure, as compared to historical rates.

It’s already helped a family as far away as South Korea. In 2011, LMH launched an online course to help moms and dads prepare for baby’s birth. One American couple overseas took the course last fall when they had difficulty finding an English-speaking obstetrician who would answer all their questions. To them, the LMH e-course was a “saving grace.” Locally, it’s more about convenience. With learning modules on pregnancy, childbirth, postpartum recovery and newborn care, parents can learn at their own pace in the comfort of their homes. The high-tech,
