Topics include: threading the machine, common seams, selecting a pattern, purchasing fabric, notions and other supplies, laying out and cutting a simple pattern, following pattern instructions, sewing on a button, and hemming a garment by machine or hand. Students need to bring a portable sewing machine to class. Emergency Medical Technician EMT Programs EMERGENCY MEDICAL TECHNICIAN (EMT)

EMT Basic v.4 | HL001-0011

The EMT/Paramedic program at EOC Tech trains students to work in Emergency Medical Services. EMT’s administer emergency care and medication to trauma victims and those who suffer sudden illness and attacks. EMT’s work with victims at the scene and during transportation to hospitals. EMT’s usually function in teams of two and may be members of hospital emergency departments, intensive or coronary care units, rescue squads or volunteer departments. Class costs include: books, CPR training, clinical shirt, FISDAP user I.D./access and SurScan Drug Screen. Costs not included are navy blue EMT pants, black boots, OSBI background check ($19), Group One background check ($45) and immunizations (cost will vary depending on where student obtain vaccines ranging between $10 to update a single TB test, or up to $390 for complete immunizations if student lacks all). Classroom uniform shirts $11 each.

Required Immunizations - Due on/before the First Day of Class • MMR (need two shots before age 18 or one after age 18), • Varicella immunization or Titer. Physician’s documentation of having had disease (Chicken Pox) is NOT acceptable • TWO Tuberculosis (PPD) tests within the last year (must be current through clinical rotations). ENROLLMENT Enrollment Begins December 1, 2014 Minimum age for enrollment is 16. The form on pg. 24 may be used to enroll. Early enrollment is encouraged, low class enrollment may cause cancellation. Classes must have a minimum of eight full tuition paying students enrolled by the week prior to the class start date. Students are not officially enrolled until tuition is paid in full.

Eastern Oklahoma County Technology Center reserves the right to cancel a class, alter time schedules and make any other changes necessary to facilitate the enrollment process.

Sending School Personnel* Faculty/staff at Choctaw/Nicoma Park, Harrah, Jones and Luther Public schools are entitled to receive a 50% tuition discount on some short-term adult classes. Examples of classes for which tuition is NOT discounted 50% include: • Classes that lead to certification • Classes requiring 48 hrs or more training • EMT classes & ALL Health classes • ALL Security/Concealed Handgun Training classes • Education2Go • Classes with tuition of $10 or less FINANCIAL AID EOC Tech offers financial aid for the eligible programs listed in the Full-Time Career Majors section of this brochure. Note: EOC Tech participates only in the Federal Pell Grant Program. No programs qualify for any type of federal student loans or work study. Most of the Full-Time career majors, including both EMT & FAC, are eligible for VA Assistance. The ACD Short-Term classes are not eligible. Oklahoma National Guard Tuition Waiver Program The full-time career majors (except EMT & FAC) are eligible for tuition waiver. All adult students are required to take an assessment test prior to admission. Students are given the Test for Adult Basic Education (TABE). The TABE measures academics of reading, math, language, and spelling. COMPASS testing is also available. ADMISSION/ENTRANCE REQUIREMENTS HIGH SCHOOL STUDENTS

Admission Requirements Contact your high school counselor about enrollment at EOC Tech. Juniors and seniors at Choctaw, Harrah, Jones or Luther schools may enroll during regular high school enrollment periods and receive credit toward their high school diploma. Students may choose from AM or PM class periods. There is no tuition charge for high school students. The procedure is as follows: 1. Talk to counselor at your school. 2. Complete EOC Tech pre-enrollment form with either sending school or EOC Tech counselor. 3. Watch for postcard sent out in May announcing the “Summer Up” schedule for June. 4. Attend “Summer Up” in June to complete paperwork and finalize enrollment. 5. Students are not enrolled at EOC Tech until enrollment is finalized at “Summer Up”. 6. Programs fill up quickly. Missing “Summer Up” may jeopardize being able to enroll in the program of choice indicated on the pre-enrollment form. HOME SCHOOL STUDENTS Students being home-schooled or attending private school must be at least 16 on or before November 1st in the year of enrollment to be admitted and allowed to enroll at EOC Tech. Students may attend in the AM or PM and will be considered as high school students. Transportation is the responsibility of the individual and must be in compliance with established guidelines and policies of EOC Tech.

Students will need provide sufficient evidence of participation in a home-school situation and meet the following admission requirements: 1. Residence/Tuition Agreement – an applicant residing in the EOC Tech district is not charged tuition for the duration of that residency. An applicant residing outof-district may be accepted pending career major availability, and may be required to pay tuition. 2. Provide a current copy of immunization records. 3. Meet the prerequisites established for the program. 4. Provide a valid transcript; or provide a written curriculum outline for the course of study provided to the individual two years prior to application for admission or duration of home-schooled time, if less than two years. 5. Provide a report of mid-term and/or final test scores for the course of study which must include formal class hours per week in which the applicant has participated. 6. Provide standardized test results from a nationallyrecognized standardized assessment administered within the last 3 years. Other assessments may be considered. 7. Once admission requirements have been met, the student will complete an enrollment application and enrolled in the program of choice if there is space available.

ADMISSION/ENTRANCE REQUIREMENTS FULL TIME ADULT STUDENTS Adult students interested in enrolling at EOC Tech must contact a Student Services Advisor to discuss program availability and career advisement. EMT and Fire Academy have special admission and enrollment procedures. Please contact Cathy Dawson at 405.390.5315 for more information. The FT Adult admission and enrollment processes are as follows: 1. Schedule an appointment to take an assessment test. Dates are limited and are usually on Wednesday’s. 2. Begin the application process for financial aid, e.g. FAFSA, VA, Tribal, etc., in order to have required documentation ready at time of enrollment. 3. Contact Student Services and schedule appointment with Student Services advisor to go over assessment testing and career major choices. a. Bring a copy of High School Diploma, High School Transcript, or GED. b. Complete Adult Student Information form to complete the pre-enrollment process. 4. Adult students will be sent a letter in June notifying them of the Enrollment Intake dates in July. 5. Students will need to call Student Services to schedule their Enrollment Intake appointment. 6. Students should finalize their financial aid paperwork before the Enrollment Intake date. 7. Students will meet with the Adult Student Advisor on that date to finalize the student’s enrollment and to discuss how the student will pay tuition. 8. Students will pay tuition not covered by funding agency or FAFSA. 9. Provide evidence of all previous education and/or training. All previous education and/or training is evaluated and given credit when appropriate. 10. Students who pre-enroll for the fall semester receive a letter about Orientation and the first day of classes. Students no longer enrolled or attending high school must be at least 16 years of age and have the permission of their parent/guardian to enroll as an adult student. A signed consent from the high school principal of the school district in which they reside is also required. However, the student is not eligible for financial aid from EOC Tech. CAREER MAJOR TUITION & OTHER COSTS ALL COSTS MAY BE SUBJECT TO CHANGE TO FACILITATE THE INSTRUCTIONAL PROCESS. Career Major Tuition Charges For all career majors (except EMT and FAC), adult students enrolled full-time (FT) are charged up to $1,080 for the school year. Adult students enrolled half-time (HT) are charged up to $540 for the school year. Charges to the student account may vary due to “open-entry enrollment” (for career majors other than EMT and FAC). Generally, the student is charged by semester ($540 for fulltime, $270 for half-time) up to the tuition cost listed above. If the student begins after the semester start date, the charge will be prorated to the nearest 1/2 quarter. If the student needs more time to complete the major, the student will be charged based on the amount of additional time recommended by the instructor.

Career Major Payments New! Students may now make any payments to Accounts Receivable/Bursar located directly across from Student Services. Students may make payment arrangements at the time of admission with the Student Services Coordinator. Adult students are responsible for payment arrangements through self-pay, financial aid, EOC Next Step Scholarship, tribal assistance, and/or outside agencies. Failure to comply with the payment schedule may result in withdrawal from the career major. Students receiving tuition assistance from an outside agency are allowed to defer payment of career major charges upon receipt by the Financial Aid Coordinator of an authorization letter and/or contract from that agency. Students usually enroll in one career major at a time. Once a student completes all coursework within a career major, they may continue in a new career major. However, students must see a Student Services advisor to be enrolled in subsequent career majors in order to determine advanced standing for courses already completed and additional tuition that may be owed at that time. There are no career major costs for secondary or home school students who reside in the EOC district. Secondary students who are out-of-district and home school students who do not reside in the EOC district may be charged for career major costs based on the information listed above. Effective, July 1, 2011, Pell Grant students are granted an “extension of credit” after the Financial Aid Coordinator has received a valid Free Application for Federal Student Aid (FAFSA) from a student who is enrolled in a Pell-eligible career major. A valid FAFSA means that the student has satisfactorily completed the verification process and any conflicting information has been resolved as determined by the Financial Aid Coordinator. Career major costs are charged to students’ accounts. Payments for these charges are automatically deducted from students’ Pell Grant disbursement checks until all career major costs have been paid in full. Any remaining Pell Grant amounts are disbursed to the students as described in their Award Letters. Students who qualify for a partial Pell Grant that doesn’t cover the career major costs are required to pay the difference by the first day of class, unless payments are approved by the Student Services Coordinator at the time of admission. Other Costs For all career majors (except for EMT and FAC), required books, supplies and tools are included in the price of tuition and remain the property of EOC Tech. Students are responsible for the care of all school-owned property assigned to them. All students may be required to pay for replacements if items are lost or stolen. EOC Tech does not offer housing or board for its students. All students live off-campus and commute. Transportation on an EOC Tech bus is available from each of the four district high schools for secondary students. Adult students may be allowed to ride the EOC Tech bus with approval from the sending school principal, and are expected to conform to the standards and regulations established for the general welfare and safety of all students. EOC TECH WITHDRAWAL AND REFUND POLICY WITHDRAWAL POLICY The Official Withdrawal Form is available from Student Services and may also be accessed from EOC Tech’s website. Cancellation of enrollment occurs when a student officially withdraws before the official start date of the program. For the Fire Academy (FAC), the Pre-Academy Orientation class occurs before the official start date of the Fire Academy program.

EOC TECH TUITION REFUND POLICY (FAC only) The school date in which the student submits the completed Official Withdrawal Form to the Fire Coordinator will be the date used to calculate a tuition refund according to the following Refund Policy (there are no refunds for books or fees): •

Students, who officially withdraw on or before the PreAcademy Orientation date, will receive a refund of all payments and all charges will be cancelled.

Students must submit a completed Official Withdrawal Form to the Student Services Coordinator before the official start date of the program. Students who officially withdraw before the official start date of the program will receive a full (100%) refund of tuition and the deposit paid.

•

Students, who officially withdraw after the Pre-Academy Orientation but before Day 1 of instruction, will receive a refund of all payments minus the $500 deposit, and all remaining charges will be cancelled.

An Official Withdrawal is defined as the action taken by a student who submits a completed Official Withdrawal Form to the Student Services Coordinator on or after the official start date (Day 1) of instruction of the student’s program. The date the completed form is submitted will be the date used in the calculation of any possible refund according to the “REFUND POLICY” (see below).

Students, who officially withdraw on Day 1 through Day 10 of instruction, will owe 25% of tuition charged; any amount paid for tuition above 25% will be refunded to the student. There are no refunds for books or fees.

Students, who officially withdraw on Day 11 through Day 39 of instruction, will owe 50% of tuition charged; any amount paid for tuition above 50% will be refunded to the student. There are no refunds for books or fees.

NOTE: Students who received the Federal Pell Grant should contact the Financial Aid Coordinator before withdrawing from EOC Tech. An Unofficial Withdrawal is defined when a student fails to attend or ceases to attend their program without “officially withdrawing” from EOC Tech (see “OFFICIAL WITHDRAWAL” section above). Non-attendance does not constitute a drop or withdrawal; the student will not receive any refund. REFUND POLICY FOR FINANCIAL AID STUDENTS Students who received the Federal Pell Grant from EOC Tech and who withdraw, officially or unofficially, will be subject to the “Return of Title IV” regulations. The date established as the student’s official withdrawal date will be used to calculate the Return of Title IV refund. Generally, the student and/or school will have to return any amount of the Pell Grant that is defined as “unearned” of the official withdrawal date. More information may be found in the Student Handbook. NOTE: Students should contact the Financial Aid Coordinator to discuss their Return of Title IV funds calculation before withdrawing from EOC Tech. OUTSTANDING OBLIGATIONS Students will be held responsible for any remaining amount of unpaid charges on their account. The Bursar will send an invoice to the student showing the outstanding obligation and an Alert will be placed on the student’s enrollment record until that amount has been paid. Students will not be eligible to receive any completions certificates or transcripts from EOC Tech until outstanding obligations have been paid. Students will not be allowed to enroll at EOC Tech until outstanding obligations have been paid. APPEAL FOR OFFICIAL WITHDRAWAL DATE In extraordinary circumstances, such as hospitalization for an accident or illness, a student may submit a written request to the appropriate Director to request an adjustment to their official withdrawal date. These requests will be handled on a case-by-case basis.

FINANCIAL AID Pell Grant The Pell Grant is a federally funded program which provides aid to post-secondary students seeking a certificate in an eligible career major at EOC Tech. Students must enroll in an approved career major with 600 or more clock hours to be eligible. The maximum Pell Grant for the 2014-15 school year is $5,730. Federal student loans are not available through EOC Tech. Pell Grants are restricted to students who have not earned a bachelor’s degree or beyond. Eligibility is based on financial need as defined by the U.S. Department of Education. These grants do not have to be repaid as long as a student doesn’t withdraw “too early”, according to federal regulations. The 2014-15 FAFSA will apply to program enrollment(s) between July 1, 2014 and June 30, 2015. Students may begin completing the 2015-16 FAFSA after January 2015 for the upcoming 2015-16 school year. It will be based on 2014 income information. Completing the Free Application for Federal Student Aid (FAFSA) Students interested in applying for the Pell Grant must complete the FAFSA. General Information • Request a PIN at www.pin.ed.gov • Parent/Guardian may be required to request a PIN as well • Utilize the FAFSA Worksheet in order to gather all pertinent information found at www.fafsa.gov • Utilize the Data Retrieval Tool for income information • Electronic processing takes approx. 24-48 hours • Paper FAFSAs are no longer available at EOC Tech • EOC Tech’s Federal School Code - 015861

EOC Next Step Scholarships Tuition waivers, known as EOC Next Step Scholarships, may be awarded to qualified high school graduates from Choctaw, Harrah, Luther or Jones High Schools. Students should contact the Student Services Coordinator for an application. Veterans Educational Benefit Program Many full-time career majors are eligible for the following VA Education Benefit programs: • Montgomery GI Bill: Active Duty - Chapter 30 • Post 9/11 GI Bill - Chapter 33 • Dependents Educational Assistance - Chapter 35 • Montgomery GI Bill: Selected Reserve - Chapter 1606 • Reserve Educational Assistance Program - Chapter 1607 • VA Vocational - Rehabilitation - Chapter 31 The veteran student who hasn’t received VA Educational benefits needs to complete the application for benefits online at www.gibill2.va.gov. The RPO in Muskogee determines VA program eligibility. Oklahoma National Guard Tuition Waiver EOC Tech will approve a tuition waiver for members of the Oklahoma National Guard who are enrolled in our full-time career majors (not including EMT Basic, EMT Advanced, EMT Paramedic, Master Firefighter-Basic, Master FirefighterAdvanced or Master Fighter-Fire Portion), either on a full-time or half-time basis, when enrollments are available.

Student Dependency Status Students are asked to answer “yes” or “no” to any of 13 statements that apply. If a student is able to answer “yes” to one or more of the statements, he/she is considered “independent” for financial aid purposes and is not required to include parents’ income. If a student answers “no” to every statement, he/she is considered “dependent” and must include parents’ income, regardless if student lives apart from parents, and/or if parents no longer claim the student on their federal income tax form. The Financial Aid Coordinator may request supporting documentation for “yes” answers. Oklahoma Tuition Aid Grant (OTAG) Application for this state grant is automatic upon completion of the FAFSA. Certain data elements from the FAFSA are sent to the OTAG program. Students must have a FAFSA submitted by March 1, 2015 (date subject to change) and have an EFC of 1,700 or lower in order to be eligible for this grant for the 2015-16 school year. This state grant is awarded on a first come, first served basis. Students may submit FAFSAs after March 1st, but they won’t be eligible for OTAG. Otha Grimes Memorial Scholarships The Otha Grimes Memorial Scholarship is awarded by the Oklahoma CareerTech Foundation. The Foundation determines the number of scholarships and the dollar amounts awarded. The number of scholarships awarded varies every term along with the dollar amounts. Applications are given to students with documented need, i.e. FAFSA, as determined by the Financial Aid Coordinator. Students may receive this scholarship once from EOC Tech.

EOC Tech Payment Plan EOC Tech offers a payment plan to students enrolling in EMT Basic, EMT Advanced, EMT Paramedic, Master FirefighterBasic, and Master Firefighter-Advanced. It is based primarily upon need which is established by completing a current year Free Application for Federal Student Aid (FAFSA) at www. fafsa.gov. • Include EOC Tech’s federal school code (015861) so that the processed FAFSA is sent electronically to the Financial Aid Office. • You may be required to verify the information provided on the FAFSA. The Financial Aid Coordinator will determine when you have a “valid FAFSA”, i.e. all documentation requirements have been met. • Provide documentation of eligibility for any other type of financial aid, e.g. tribal assistance, to the Financial Aid Coordinator so that the correct amount of charges remaining is the amount required to be paid out by the student. • Complete the Payment Plan Application form and bring to Student Services. It will be given to the Financial Aid Coordinator. • Before your enrollment in the program, a Payment Plan Contract will be set up and given to you to review and to sign if you agree to the terms. • The general terms will be the first payment of 50% of the total cost is due up front followed by a payment of 25% each by the end of the following month and the final payment of 25% by the end of the next month. • If circumstances occur which may cause you to be unable to comply with the terms of the Payment Plan Contract, you may appeal before breaking the terms of the contract. Please refer to the appeals process in the Student Handbook. • Failure to comply with the Payment Plan Contract may result in immediate withdrawal from the program. No certifications or transcripts will be issued. Outside Agencies Who Offer Financial Assistance Additional financial assistance may be available from area programs such as Workforce Investment Act (WIA), DRS Vocational Rehabilitation (Voc-Rehab), and the Bureau of Indian Affairs. Contact these organizations for more information and how to apply for benefits. STUDENT SERVICES Navigating through the educational process can present both challenges and opportunities. EOC Tech’s Student Services staff are committed to helping students achieve their educational and career goals. Students are invited to visit the Student Services office at any time. Our goal is to assist the student and make their educational experience at EOC Tech enjoyable. PLAN OF STUDY Students will meet quarterly with advisors and their instructors to review the student’s Plan of Study sheet that was given to the student at “Summer Up”. The Plan of Study covers the following: 1. Career major information 2. Course grades 3. Workplace/College information 4. Progress towards a nationally-recognized certification 5. Enrollment options for next school year (last meeting of year)

REGISTRAR

The Registrar’s Office issues official transcripts and certificates. There is no charge for these documents. Students must request these documents by completing the appropriate form described below. These documents may require time for records to be researched and verified; students should allow 5 business days for requests to be processed. Contact the program secretary for an enrollment confirmation or unofficial transcript. Request for Enrollment Confirmation/Transcript/Certificate •

Enrollment Confirmation Enrollment status, tuition costs and scheduled class times can be verified by letter. The letter is addressed and mailed to the business and/or agency requesting the information. A copy of the letter is sent to the student/parent upon request. Unofficial Transcript Completed course names/grades printed on EOC Tech letterhead without Registrar’s signature. Official Transcript Completed course names/grades printed on official paper with Registrar’s signature. Certificates Issued based on the following: 1. Career Major Completer Every course is completed with course grade within the career major. 2. Course Concentrator At least 360 hours or more completed with course grade within the career major. 3. Course Completer At least one course, but less than 360 hours is completed with course grade within the career major.

Information will not be released if the student has outstanding financial obligations to EOC Tech. Official transcripts cannot be faxed due to the security paper safeguards of the official transcript paper. Please contact the Registrar’s office for more information.
