SELF SELECTION OF P MEDS

Assertions from the pharmacy regulator that the self-selection of P medicines should be progressed have been opposed by a new petition from pharmacy employee representatives. As July drew to a close, pharmacy regulator General Pharmaceutical Council (GPhC) repeated its commitment to modernise pharmacy through the self-selection of P medicines. It has reasserted its rationale in that a change in P medicines sales protocols can be supported by pharmacy processes that ensure patients receive the appropriate sales advice. However, in response to the regulator’s proposals, the Pharmacists’ Defence Association (PDA) has launched a petition seeking withdrawal of the proposals. It has promised that this is “just the start of [a] full campaign to seek a change in the current GPhC policy” which will be stepped up this year and beyond.

Progressing P medicine As the proposed new protocols will not be obligatory, it is, perhaps, also true to say that the GPhC is demonstrating trust in its registered pharmacy professionals; that as responsible, accountable professionals, they will act according to the recognised limits to their competence. Given that pharmacy thinks of itself as a profession that can support patients with medicines optimisation and, as Pharmacy Voice has recently suggested , take a lead in the care of patients taking high risk medicines, the GPhC’s thinking on this matter is all very reasonable – is it not? Well, no, it isn’t, say the critics of the GPhC’s proposals, which include the PDA and the Royal Pharmaceutical Society (RPS), which is expected to comment on the GPhC’s missive in the near future. From the outset, the PDA has been among those to oppose the GPhC on this matter, including making its opinions known during a Keynote theatre question time at Supported by

Explaining its opposition to the GPhC’s proposals, the PDA believes the current P medicine sales promote interventions that protect patient safety. “Should [they] be removed [this will] make the pharmacists’ role of ensuring that the public get the right P medicines significantly more difficult to deliver.”

Among its specific concerns, the PDA highlights the potential for: • Greater tension between commercial and patient safety considerations in community pharmacy • Increased amount of medicines in circulation • Public confusion • Harming POM-P de-regulation processes. The employee representative says: “The profession is now faced with the prospect of a new regulator that lacks expertise in pharmacy, proposing a fundamental change in practice despite formal protestations and warnings about patient safety. Additionally it denies the public and practitioners those that these changes will affect - a consultation on this matter. These behaviours are a cause of great concern, they do not represent transparent modern regulation and these are questions that the regulator must answer.” Continued on page 4

Are we selling our souls Over The Counter? Blog by @SparkleWildfire There’s something beautifully logical and satisfying about a good Over The Counter consultation. I know what you’re thinking, and yes, I should get out more. But it is true. The way I think of it is in a stepwise approach, using a systematic series of questions to eliminate products leaving you with a limited number of options to recommend. I suspect the way a patient views it may vary somewhat from “hey! I’m really being listened to!” to “oh for goodness sake shut up, I’m going to be late for the bus.” Continued on page 2

Are we selling our souls Over The Counter?

Once I know what the problem is, I think about what products are available to treat it over the counter. STEP

Find out about the patient’s medical history.

I’m looking for anything that means the patient is at high risk of complications of the problem, meaning they need to be referred. I’m also thinking of the cautions and contraindications of the available products to treat the problem and eliminating any products that these apply to. STEP

Find out about the patient’s drug history.

I’m thinking about what drug interactions each of the available products has, and whether or not they are relevant to what the patient is already taking. I’m also thinking about whether or not any of the patient’s medicines contain the same or similar ingredients to any of the OTC products- i don’t want to risk an overdose. I then eliminate any products that have relevant drug interactions. STEP

Consider the evidence.

At this point, there may be only a couple of products that are suitable for the patient. If there aren’t any left, I refer them to their GP or appropriate healthcare provider. Out of the ones that are left, I think about which ones the evidence says are more effective and safe and I’ll steer the patient towards that one. If there’s no good evidence for a treatment, I’m

Where two products are equally efficacious, I always recommend the cheapest. Because I don’t want to waste a patient’s hard earned cash, and I think patients appreciate that. STEP

Counsel the patient.

This involves giving them all the useful little snippets of information about how to use the product effectively and -crucially- safely. It might be pointing out that the product contains paracetamol, or that it causes drowsiness, or that it works best taken at a particular time of day. So why am I on about this at the moment? One reason is the Which? report, which has found a disappointing quality of over-the-counter information being provided in some pharmacies. However, it may well be indicative that in a world where pharmacists are under massive pressure to deliver services and dispense ever-increasing numbers of prescriptions whilst working longer hours for less pay, some are starting to lose pride in the basics. I’m not saying this is what is happening in every pharmacy or in every case, but based on how I used to feel when I managed a pharmacy, I suspect it is a factor. The other reason is the issue of self-selection of pharmacy medicines. In my humble opinion, this is a giant backwards step for the pharmacy profession. Self-selection reduces the process I described above down to a bare minimum, and reduces the possibility of introducing quality into it. Sure, we can say there will be information on the packs, but let’s be realistic about this. Patients very rarely read the leaflets before purchasing, and in my experience, if a patient has a preconceived idea of the product they want to buy, it’s very difficult to persuade them otherwise, even when there is a safety concern with their chosen product. I remember one customer asking for Sudafed, when she was also taking phenelzine. A combination of these drugs could

Why would the patient bother reading the leaflet, when she already thinks it’s safe for her to use? And so, what appeared to be a quick, convenient shop for the customer could so easily turn into a fatal drug interaction. The wares we sell can have fatal effects if they’re not used in accordance with expert knowledge and instruction, we simply cannot safely reduce the over the counter consultation to the bare minimum of words required to take money from people as quickly as possible. Of course, it’s not like we pharmacists, will get that much say in the matter, and we’ll carry on regardless if self-selection goes ahead. Indeed it seems to be a near certainty, despite support from “very few” pharmacists. And it may even be the case that such scenarios never happen in real life. But all I want to do is make medicines safer for people, and to do so we have to identify areas of risk before they happen, and take action to minimise them. When it comes to patient care, just hoping the worst doesn’t happen isn’t good enough.

Now imagine this situation in a self-selection age. The customer picks up the pack, wanders to the pharmacist, who then has to try to wrangle it from their fingertips in order to ensure their safety. Okay, maybe not that dramatic, but the fact that the customer has the product in their hand, and has been able to “choose” it themselves, reinforces to them that it is their right to buy it. But in pharmacy, the fact of the matter is that the customer isn’t always right, and its poor customer service- and downright dangerous- to always allow them to think so.

Pharmacy Voice data highlights community pharmacy’s safety role in high-risk medicines pathway Figures released by Pharmacy Voice, which represents community pharmacy owners, highlight the contribution of pharmacists to the detection of patients at risk of adverse effects from medicines which require close monitoring. The report highlighting pharmacists’ role in medicines and patient safety is a timely in view of the introduction of a major new twoday conference at the Pharmacy Show (around medicines safety and optimisation) in September, chaired by former National Clinical Director Jonathan Mason (pictured). Audit data collected from 2,773 pharmacies in England and Wales found that just under 14% of patients taking methotrexate (to treat arthritis), lithium (to treat bipolar disorder) or warfarin (to thin the blood) are not having their regular blood tests which are used to check they are receiving the correct dose. In the course of the audit, community pharmacists identified 425 patients who were showing signs of toxicity. Patients who are prescribed warfarin, methotrexate or lithium are usually given a Patient Record Booklet which healthcare professionals use to schedule blood tests and to ensure the correct strength of the medicine is prescribed.

The Pharmacy Voice research found that of all the patients surveyed in this study only about a third of them brought the booklet with them when collecting their medicine.

In the light of the findings, Pharmacy Voice (PV) is renewing its call for pharmacists to have access to electronic patient records. PV believes this would enable pharmacists to use clinical information about a patient’s treatment more easily so that patients can be effectively monitored and referred for early investigation of issues with medicines if necessary.

Using community pharmacy to fight flu? By Mark Robinson, Medicines Management Partnership Last year was another quiet season for England, however, some other countries experienced more severe flu. This must remind us that flu can be dangerous and highly unpredictable. If we have a more active season, it could be the death of many small and frail companies within our fragile economic growth and a very severe test to the NHS and social services. The target is to reach or exceed 75% uptake for all groups. This is really going to be a stretch since the immunisation in the aged under 65 at risk groups have stalled at 50% for a couple of years now. The number of immunisations given last year hardly increased by much over the previous year. And we missed the 75% WHO target for the over 65s for the seventh consecutive year. Last year the NHS delivered an additional 274,279 immunisations to defined at risk groups to virtually stand still. That is hardly surprising at the rate that we are discovering long term conditions. This year, how many additional immunisations are required? If you assume that we need an additional 300,000 to stand still, an additional 1.5 million to hit targets and perhaps 200,000 depending on the immunisation program in pre-school children – then it is a staggering 2 million more and 15%-20% more than last year. We are told that we need ‘new strategies’ to deliver these levels of achievement, but NHS England and Public Health England believe that the new structures should ‘enable innovative approaches to improving flu vaccine uptake’. For the last few years some community pharmacies in some areas have been commissioned to provide flu vaccines. They have a unique role in picking up the under 65s who visit

the community pharmacy regularly to pick up LTC medicines, but see their GP practice nurse just once a year. In general this has been a success; community pharmacies have been recruited, pharmacists have been trained, MCAs have been trained and many pharmacies have made real contributions to the target. I would say that they are picking up speed and will go from strength to strength. Perhaps half of the additional requirements could be picked up by community pharmacy this year. We are told that: • Public Health England will provide a centre of expert advice and monitoring • Directors of Public Health in the Local Authority have a duty to ensure plans are in place to protect their population through screening and immunisation. They will bring together key stakeholders to develop multi-agency approach to achieving the targets in their local area. They will inform NHS England of the commissioning requirements. They will also provide independent scrutiny and challenge of the plans that NHS England develops.

This is the time to rally around and get community pharmacy commissioned as providers of flu immunisation all around the country. Just when the NHS and the economy is under the biggest strain, the last thing that we actually need is loads more people with flu! At the current time, I know that the North East and Cumbria and Lancashire are commissioning community pharmacy, Manchester is on the way and London has decided not to. I am simply unaware of the other Area Team intentions – you would need to fill me in. The Directors of Public Health collectively have the responsibility of drawing up local plans to ensure that the additional 2 million doses are given during this season. If we fail, then the blame lies squarely at their feet. It is up to community pharmacy in areas where commissioning has not been confirmed to contact their Director of Public Health and work with them to ensure that they are included within the Area Team’s commissioning plan.

We’ve done lots of research in preparation for this year’s show to ensure we remain at the forefront of what delegates want and ensure that the Pharmacy Show continues to be steered by the industry. As a result, we’ve made a few changes and created a bigger and even more relevant event for you. Following feedback from delegates, we have restructured our education programmes to reflect the realities of the current and changing pharmacy landscape. What’s new for 2013?

Independent Pharmacy Federation Chief Executive, Claire Ward (pictured) has called on the government to ‘Think Pharmacy’ as it responds to the decision of NHS Direct to withdraw from contracts to supply services to the NHS 111 helpline.

• We are really excited to be hosting a Special ‘town hall’ briefing and 1-2-1 advice clinics by the General Pharmaceutical Council (GPhC) on how the regulator plans to work for and with patients to improve standards in pharmacy.

Consideration is already being given to how GP out-of-hours services can pick up some of the loss from NHS Direct. GP leaders have called on the government to let them provide additional care.

• We have a two-day conference stream on Medicines Optimisation, chaired by Jonathan Mason, Clinical Adviser (Medicines), NHS England (London Region).

But Ward, a former Labour MP for Watford, says that pharmacy can be part of the solution: “The reorganisation of NHS Direct to NHS 111 direct has been a disaster. Across the country we will now see people worried about where to turn when they need advice.

• Further 1-2-1 drop-in advice clinics will be taking place with leading independent pharmacists on running services for ‘flu vaccinations, travel clinics, mobility, weight management and smoking cessation, hosted by the Independent Pharmacy Federation (IPF). • The four major interactive keynote debates on pharmacy’s future in the new NHS will give delegates the opportunity to listen to different perspectives from professional leadership bodies, independent entrepreneurs, the bosses of the multiples and other primary care leaders – and have a say where they see fit. • Then our new-look two-day clinical conference stream, which has been programmed in association with our sister event, The Clinical Pharmacy Congress, focusses on eight therapeutic areas, with interactive patient case studies - a UK first! The 2013 Pharmacy Show is set to face the latest challenges in pharmacy head on through education – you will gain vital knowledge in increasing footfall, increasing basket spend and ultimately increasing cash flow and profitability!

“We know the hospitals cannot cope with added demand as a result of the loss of confidence in the helpline. As the government considers what to do next it should be thinking about how to involve community pharmacy in its plans.” She said that community pharmacy can deliver services to patients with minor ailments, supports those with longterm conditions, gives access to emergency medication and other patient concerns. She believes that promoting these services in partnership with GP out-of-hours services would be an efficient way to deliver support to the public and minimise the number of people who will arrive at A & E when they do not need to. The future of pharmacy’s role in the NHS will be hotly debated by all stakeholders in the profession in a series of four special debates at the Pharmacy Show on September 29th and 30th at the NEC. The Pharmacy owner Fin McCaul will be representing IPFs views on the professional leadership panel in the debate.

Open display of pharmacy medicines: what the GPhC says... The General Pharmaceutical Council (GPhC) has set out clearly what it has decided about the open display of pharmacy medicines or self selection.

Self selection is not allowed at this time in Great Britain The law says that the sale of pharmacy medicines must be supervised by a pharmacist and the law has not changed in this regard

Self selection will not be allowed in Great Britain until key safeguards are in place. No date has been set for that yet

Any pharmacy wishing to introduce self selection will have to assure the GPhC that it can be done safely, before self selection will be allowed

The GPhC will not be requiring pharmacies to introduce self selection; pharmacies will have a choice whether they introduce it and that choice will bring clear responsibilities, like keeping patients safe
